color: rgb(0, 0, 0);\"><strong>Job Description</strong></span></p><p><span style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">The Medical Social Worker is responsible for the implementation of standards of care for medical social work services provided by the agency.&nbsp;</span></p><ul><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Identifies and assesses the social, emotional, environmental, financial and support factors underlying patient needs</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Assesses the psychosocial status of patient&rsquo;s related to the patient&rsquo;s illness and environment and communicates finding to RN Case Manager</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Communicates patient status regarding social and emotional factors to a team of clinicians</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Carries out social evaluations and plans intervention based on evaluation findings</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Counsels patients on how to overcome their condition and avoid dependencies as they recuperate from their illness</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Provides crisis intervention, emotional support and resource information when necessary</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Maintains and updates clinical records on all patients referred to social work and prepares clinical and progress notes to share with the clinical team</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Educates patients and families on, and assists in, preparation of advanced directives</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Serves as the liaison between patients and families/caregivers and community social service agencies&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Participates in the development of the individualized plan of care and attends case conferences&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Partners with public and private health insurers to determine the patient&rsquo;s benefits and advocate for the patient</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Participates discharge planning</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Performs other duties as delegated by the Clinical Director</li></ul><p><span style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\"><strong>Basic Qualifications</strong></span></p><ul><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Master&rsquo;s degree in social work from accredited school of social work&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">At least two years&rsquo; experience in a health care setting</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">&nbsp;Licensed clinical social worker with the Indiana Professional Licensing Agency preferred</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Ability to network with community agencies and utilize community resources</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0);\">Ability to make evaluations and decisions based on measurable or verifiable criteria</li><li style=\"font-family: tahoma, sans-serif;
