font-size: 14px;\"><br></span></p><p><span style=\"font-size: 14px; font-family: arial, sans-serif;\"><strong><u>POSITION SUMMARY</u></strong></span></p><p style=\"margin-left: 20px;\"><span style=\"font-size: 14px; font-family: arial, sans-serif;\">Serve as the hospice expert to professionally and seamlessly coordinate services and bridge the transition for the patient from the hospital to hospice services; develop long-term relationships at each facility to reinforce CHOT/Providence Hospice as the provider of choice.&nbsp;</span></p><p><span style=\"font-size: 14px; font-family: arial, sans-serif;\"><strong><u>ESSENTIAL FUNCTIONS</u></strong></span></p><div style=\"margin-left:0in;\" data-pasted=\"true\"><ul><li>Respond to referral sources promptly; evaluate patient appropriateness for hospice; meet with families/patients to discuss goals and explain services of hospice</li><li style=\"margin-left:0in;\">Complete required paperwork with patient/families; facilitate patient discharge by working with hospital staff, DME, medications, hospice physicians, and hospital staff</li><li style=\"margin-left:0in;\">Educate hospital staff and follow up on pending patients; round daily to be available to staff and patients for questions and to build relationships</li><li style=\"margin-left:0in;\">Communicate and provide daily feedback to admission staff on referrals</li><li style=\"margin-left:0in;\">Complete thorough documentation of hospice eligibility in the required timeframe</li><li style=\"margin-left:0in;\">Through customer service and leveraging relationships, become the &ldquo;hospice of choice,&rdquo; thereby increasing hospital referrals</li><li style=\"margin-left:0in;\">Attend and participate in routine staff meetings in person or via media provided as directed</li><li style=\"margin-left:0in;\">Participate in and/or provide in-service training, attend various hospital activities, and perform other duties as required or assigned</li><li style=\"margin-left:0in;\">When a patient is admitted to GIP, complete an initial, comprehensive assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illness(es)</li><li style=\"margin-left:0in;\">Assess and evaluate patient&#39;s status by:<ul><li>Writing and initiating a plan of care</li><li>Regularly re-evaluating patient and family/caregiver needs</li><li>Participating in revising her plan of care as necessary</li></ul></li><li style=\"margin-left:0in;\">Initiate the plan of care and make necessary revisions as patient status needs change; include the patient and the family in the planning process</li><li>Complete, maintain and submit accurate and relevant clinical notes regarding patient&rsquo;s condition and care given. Record pain/symptom management changes/outcomes as appropriate</li><li>Communicate with the physician regarding the patient&rsquo;s needs and reports changes in the patient&rsquo;s condition; obtain/receives physicians&rsquo; orders as required</li><li>Communicate with hospice staff to coordinate the care plan</li><li>Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family</li></ul></div><p><span style=\"font-size: 14px; font-family: arial, sans-serif;\"><u><strong>POSITION QUALIFICATIONS</strong></u></span></p><ul><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Registered RN in the State of Texas required &nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Nursing experience or medical training required (ability to read charts, know medications, etc.)&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Marketing experience required; 1-3 years of experience in healthcare marketing preferred&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Hospice experience strongly preferred&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Proficient computer skills are required</li></ul><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\"><strong>Reasonable Accommodations Statement&nbsp;</strong></span></p><p style=\"margin-left: 20px;\"><em><span style=\"font-family: arial, sans-serif; font-size: 14px;\">To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
