Description

Scheduled Work Times/Locations Monday – Friday, 8:30AM – 4:30PM; flexibility for occasional nights and weekends when necessary Location: 118 Main Street Hartford, CT The Team Supervisor is responsible for the overall function and implementation of the programs housed at St. Elizabeth House (Women’s Recovery, Aids Residential, Respite, etc.), ensuring the highest quality of service within the mission of Mercy Housing and Shelter Corporation. This individual will carry a caseload in addition to providing guidance, assistance, and supervision of all Case Managers and Employment Specialists, while also serving as support to the Director of Client Services. Other overall position responsibilities include but are not limited to, maintaining cooperative relationships with community agencies and partners, participating in community efforts per Director’s direction (GH CAN meeting, etc.), facilitating case management meetings on a regular basis, including scheduling and documentation of meeting notes. Supervisory Duties and Responsibilities: Manage workflow of case managers including defining goals, communicating objectives, and monitoring performance. Review resumes and interview candidates to fill vacancies. Train new hires in all aspects of case management. Train new hires, monitor data input and integrity of data. Create and maintain team schedules including approving PTO and timecard exceptions. Provide regular supervision to case managers and employment specialists while providing documentation and feedback. Oversee completion of required documentation within the prescribed timeframe per funder and agency policy case notes, and case files of subordinates. Complete formal employee evaluations on an annual basis. Work with Director and HR on staff morale and office environment issues/improvement Documentation of any employee disciplinary/performance issues Case Manager Duties and Responsibilities Program specific; have or develop a strong expertise providing housing coordination services for eligible clients, provide training to clients regarding those housing opportunities and regarding budgeting and maintaining a household, and other skills that are deemed necessary to successfully achieve stability in permanent housing. Provide trauma informed care to domestic violence and other trauma survivors. Ensure clients have the necessary documents to access services, including Social Security card, official picture ID, birth certificate, proof of citizenship, and proof of income eligibility. Complete all required documentation and case notes and maintain case files. Input data into the HMIS system. Warm-transfer clients to other agencies and ensure necessary support and services are in place if clients need or request longer-term case management or on-going services. Engage clients by building trusting and professional relationships. Coordinate client intake processes as directed by Director of Client Services. Work with clients to develop individual service plans with measurable goals. Oversee the development, implementation, coordination, and monitoring of the service plan. Provide case management, crisis management, assistance, and education in activities of daily living, and services toward stabilization of the family unit. Ensure regular face-to-face and collateral contacts with clients, providers, family/guardians, and other team members. Mediate conflicts between providers, family/guardians, clients, and the agency, per the client’s needs. Follow proper discharge procedure when a client is discharged, with attention to recordkeeping and referrals. Transport clients as needed. Travel and network in the community. Maintain complete and up-to-date client files to assure overall compliance with agency standards. Participate in ongoing professional development. Implement MHSC policies/procedures. Perform any other task that will assist the agency in carrying out its mission. Required Skills/Abilities: Demonstrated knowledge of trauma informed crisis intervention, domestic violence, child abuse/neglect, stalking, elder abuse intervention; experience in Crisis Counseling. Self-starter, able to work and make decisions independently. Excellent interpersonal skills, strong ability to set boundaries with clients. Ability to collaborate with community organizations and Supervisor. Excellent verbal and written communication skills including in virtual settings. Excellent organizational skills and attention to detail. Excellent time management skills: ability to prioritize tasks. Strong analytical and problem-solving skills with a can-do attitude. Ability to lift boxes and other materials up to 20 pounds. Ability to adapt to the needs of the organization volunteers, and grant requirements. Education and Experience: Bachelor’s degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field. Have a familiarity with housing first, progressive engagement, and reality testing. Experience with conflict resolution and mediation. Five (5) years relevant work experience and demonstrated ability to provide effective services to clients required. Two (2) years supervisory experience needed. Needs to be culturally competent across all populations. Bilingual a plus. Licenses and Certifications Must have valid Connecticut driver’s license and clean driving record with proof of automobile insurance. Must have a vehicle in good working condition. Compensation & Benefits $28.00 - $31.00 per hour, based on experience Community Housing Advocates provides a generous employee benefits package, including medical, dental, retirement, short- and long-term disability, and tuition reimbursement. Full-time employees receive 10 days of PTO (paid time off) per year, plus 11 paid holidays and one (1) Floating Holiday and 40 Hours of Accrued Paid Sick Leave.
