Description

Job Title:  Case Manager FLSA Status: Non-Exempt Reports to: Director of Survivor Support Services Job Purpose Provide trauma-informed case management services to guests of YWCA Shelter. Primary Duties and Responsibilities Serve as the primary Case Manager for women with children. Provide all services in accordance with a woman-defined service model, and MOCADSV service standards. Complete strengths/needs assessments and housing goal plans with assigned guests. Provide information, referral and advocacy to assist guests with accessing mainstream health and human services programs. Coordinate and facilitate the provision of services provided by other organizations or professionals. Provide transportation, as needed, to assist with guests with accessing services and community resources. Employee must possess a valid driver's license and insured vehicle with insurance complying with minimum agency standards. Maintain confidentiality of all service guests in compliance with state and federal law and YWCA policy. Conduct safety planning with all assigned guests and review as needed. Provide information regarding Crime Victim’s Compensation and assist with applications as appropriate. Oversee data quality to maintain compliance with data standards. Maintain statistics and complete reports as assigned. Facilitate support/educational groups as assigned. Provide shift coverage in shelter as assigned. Attend staff meetings and trainings as required and/or assigned. Commitment to the YWCA mission of Eliminating Racism and Empowering Women. Other related duties as required and/or assigned. Qualifications Bachelor’s degree in human service field or equivalent education/experience with related case management experience preferred. Knowledge of domestic & sexual violence dynamics preferred. Knowledge of community resources preferred. Excellent interpersonal and communication skills. Ability to maintain confidentiality. Ability to effectively engage and build relationships with diverse populations. Access to private transportation, valid driver's license with insurance complying with minimum agency standards.   Working Conditions Forty (40) hour workweek with flexibility regarding days/times to meet guests' needs, shelter coverage and/or deadlines. Employee works in an office environment and is protected from weather conditions and contaminants. Ability to work flexible hours, including evenings and weekends, based on guests’ needs and business demands. Some on-call responsibilities are required. May provide transportation to guests as needed. Physical Requirements Standing or sitting for long periods, bending, stooping, and occasional lifting. Repetitive hand motion (such as typing). Ability, on a consistent basis, to perform activities requiring cooperation, instruction, persuasion, or speaking with others. Ability to communicate effectively in person with guests. Personal Characteristics The successful individual will demonstrate the following: Behave Ethically: Understand ethical behavior and professional boundaries and ensure own behavior is consistent with professional ethical standards. Build Relationships: Establish and maintain positive working relationships with others both internally and externally to achieve the organizations goals of empowering women and eliminating racism. Communicate Effectively: Speak, listen and write in a clear, thorough and timely manner using appropriate and effective communication tools and techniques. Focus on Guest Needs: Anticipate, understand and respond to the needs of guests. Make Decisions: Assess situations to determine the importance, urgency and risks and make clear decisions which are timely and in the best interests of the guest and the organization. Organization: Set priorities, develop a work schedule, monitor progress towards goals and track details/data/information/activities.
