Description POSITION SUMMARY: The PrEP/PEP Navigator will play a critical role in expanding and strengthening low-barrier access to HIV prevention services, specifically pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP), among priority populations disproportionately impacted by HIV in Fresno and surrounding regions. This person will provide education, resources, and personalized assistance to ensure clients have access to necessary healthcare services and support throughout their PrEP journey. This position will provide individualized client navigation, benefits counseling, linkage to medical care, PrEP/PEP adherence support, and coordination of social and supportive services. The PrEP/PEP Navigator will engage communities through culturally responsive, trauma-informed, and harm reduction-based approaches and will collaborate with clinical providers, CBOs, and public health partners to integrate prevention services into existing care pathways. ESSENTIAL JOB FUNCTIONS: Provide individualized client navigation, including benefits counseling, linkage to medical care, and support to ensure timely, low-barrier access to PrEP/PEP services. Assist clients with appointment scheduling, provider linkage, transportation coordination, and follow-up reminders. Offer PrEp/PEP education, adherence Support, and ongoing  follow-up through ongoing client engagement, education, follow-up, and motivational interviewing. Coordinate with benefits counselors, case managers, and partner agencies to maintain continuity of care. Coordinate with clinical providers, community-based organizations, and public health partners to integrate HIV prevention services, HIV testing, STI screening and treatment, and primary care services. Maintain referral pathways and relationships with clinical partners and community service providers. Conduct targeted outreach in community settings, events, and partner organizations to engage priority populations. Facilitate educational sessions on sexual health, PrEP/PEP, and STI prevention. Track service delivery, referrals, outcomes, and adherence support activities in required data systems. Support program evaluation efforts and continuous quality improvement. Support mpox vaccine education, outreach, and navigation (training provided). Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and Perform any other duties as assigned. Qualifications ESSENTIAL QUALIFICATIONS: To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Experience consisting of one (1) year providing similar services; Knowledge of HIV prevention strategies, including PrEP, PEP, and harm reduction principles. Understanding of sexual health, STI transmission, basic medical terminology, and local health systems. Ability to conduct risk assessments, provide culturally responsive education, and reduce client stigma or barriers to care. Strong communication, motivational interviewing, and rapport-building skills. Proficiency in documentation, data entry, and using electronic health records or case management systems. Must have the ability to pass a criminal history background screening; Valid driver’s license and reliable transportation CPR Certification, First Aid Certification and an Annual Tuberculosis test. EDUCATION and/or EXPERIENCE: Associate or bachelor’s degree in public health, social work, health education, human services, or a related field is preferred. Will accept equivalent combination of education and experience. GED or High School Diploma is required. WORKING CONDITIONS: Work is performed in a combination of office, clinic, and community settings, including outreach sites, and partner agencies, as appropriate. Frequent local travel may be required to conduct outreach, attend meetings, transport materials, or accompany clients to appointments. PHYSICAL DEMANDS: Ability to sit, stand, and walk for extended periods during office work, client interactions, and community outreach. Regular use of hands and fingers to operate computers, phones, and other office equipment. Occasional lifting, carrying, or moving of materials and supplies up to 20 pounds. Ability to travel locally for meetings, outreach events, and client appointments. Must be able to communicate clearly in person, over the phone, and in written form. May require bending, reaching, or other moderate physical activity while conducting outreach or setting up materials.
