The Triage Registered Nurse is responsible for answering questions, gathering information, assessing patient needs and when needed, coordinating and directing hospice services/care via phone conference. The Triage Registered Nurse is experienced in nursing with an emphasis on palliative care.</p>\n\n<p><strong>To be considered, candidates must reside in the following states: WI, MN, IA, IL, SD, MI, NE, MO, KS, IN</strong><br />\n<br />\n<strong>Essential Functions and Skills<br />\nPatient Care</strong></p>\n\n<ul>\n\t<li>Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing.</li>\n\t<li>Receives calls related to patient care and instructs patient/caregiver on appropriate interventions based on nursing knowledge.</li>\n\t<li>Evaluates available options for interventions and uses critical thinking when selecting which intervention is best for the patient and/or caregiver.</li>\n\t<li>Determines the need for nursing visit based on EMR and telephone call. Dispatches a visit RN or LPN as appropriate.</li>\n\t<li>Completes, submits, and maintains accurate and relevant clinical notes regarding patient&rsquo;s condition and care given. Records pain/symptom management and changes/outcomes as appropriate.</li>\n\t<li>Communicates with the physician regarding patient needs and reports changes in patient condition; obtains/receives physician orders as required.</li>\n\t<li>Facilitates the patient&rsquo;s efforts toward self-sufficiency and optional comfort care.</li>\n\t<li>Counsels and works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.</li>\n\t<li>Ensure arrangements for equipment and other necessary items and services are available.</li>\n\t<li>Responsible for maintaining compliance with all HIPAA regulations and requirements.
