    "requisitionDescription": "<div><div><div><div><p id=\"isPasted\"><strong><span style=\"font-family: arial, sans-serif; font-size: 14px;\">JOB RESPONSIBILLITIES:</span></strong></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Conducts self ethically in accordance with all relevant standards and regulations and demonstrates respect for patient rights at all times.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Works with an interdisciplinary team to develop and implement a treatment plan of care for patients based on a thorough assessment of the patient&rsquo;s symptom presentation, professional/referral input and all other available data.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Participates as an active member of the Referral Development Team. Appropriately participates in referral development processes and activities as directed.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">&nbsp;Assesses patient progress and communicates results and trends to supervisor and treatment team.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Conducts group and individual and family therapy sessions in accordance with patient&rsquo;s diagnosis and needs and in accordance with state/regulatory standards.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Conducts activity therapy sessions and educational group sessions as directed.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Provides ongoing education to patients and families.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Provides appropriate resources and support for patients before, during, and after treatment.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Engages in timely, efficient discharge planning. Works in close conjunction with patient family, nursing staff and attending physician.
