Description The Opportunity: We are seeking to hire a Case Manager – ACT Team. This individual provides ongoing community psychiatric support treatment services for a select group of clients who have a serious and/or persistent mental illness co-occurring with substance abuse disorder. This Case Manager will provide training, assistance, and support to help this client receive wrap-around rehabilitation services in the community. Our ACT Teams work together, meeting frequently, to facilitate an integrated, multidisciplinary team approach to service regarding progress, general problems, and concerns pertaining to clients served and their community adjustment. What You’ll Do: Collaborate with the client and the Service Coordinator to develop treatment plans that shall include services and assistance necessary to achieve recovery goals, including an explanation of the availability, intensity, and duration of each service. Implement programming according to the developed treatment plan, participating in the ongoing assessment and therapeutic engagement of ACT team clients. Make observations and document progress in accordance with established agency procedures. Assist clients in developing daily living skills, including teaching and assistance with medically necessary services such as laundry, cooking, cleaning, communication and social skills, recreation, grocery shopping, personal hygiene, and self-care skills. Monitor medications in accordance with agency procedures to ensure that proper dosages are taken as prescribed. Assist clients in the proper budgeting and use of their funds, acting as an advocate for clients to secure needed services and financial entitlements. Perform on-call duty and weekend coverage on a rotating basis. Maintain relevant documentation. This position is full-time (40 hours a week) working normal business hours. Located in downtown Akron, Ohio, we serve as a leader in behavioral health care for the entire Summit County area. What We’re Looking For: High School diploma required, bachelor’s degree preferred. Non-Bachelor’s candidates must be certified as an Ohio Certified Peer Support Recovery Specialist or qualified mental health specialist (QMHS). A minimum of three years of experience working with clients with a severe and/or persistent mental illness and/or substance abuse preferred. LSW, LPC, LPN, CDCA, LCDCII, or LCDCIII preferred. A car and a valid Ohio driver’s license and insurability through the Agency’s insurance carrier required. A deeply held belief in CSS’s recovery-centered mission. Ability to develop positive relationships with individuals with mental illness, developmental disabilities, and substance abuse challenges. Skill in assessing individuals for strengths, needs, abilities, and preferences. Excellent organizational, attention to detail and communication skills. Ability to work in a team environment. Proficiency in Microsoft Outlook. Skill in managing time and meeting deadlines. Ability to negotiate assertively from an advocacy perspective. Knowledge of community resources and demonstrated ability to access. Ability to work a flexible, varied schedule. This Case Manager will provide training, assistance, and support to help this client receive wrap-around rehabilitation services in the community.&nbsp; Our ACT Teams work together, meeting frequently, to facilitate an integrated, multidisciplinary team approach to service regarding progress, general problems, and concerns pertaining to clients served and their community adjustment.\\r\\n\\r\\n&nbsp;\\r\\n\\r\\nWhat You&rsquo;ll Do:\\r\\n\\r\\n\\r\\n\\tCollaborate with the client and the Service Coordinator to develop treatment plans that shall include services and assistance necessary to achieve recovery goals, including an explanation of the availability, intensity, and duration of each service. \\r\\n\\tImplement programming according to the developed treatment plan, participating in the ongoing assessment and therapeutic engagement of ACT team clients.\\r\\n\\tMake observations and document progress in accordance with established agency procedures.\\r\\n\\tAssist clients in developing daily living skills, including teaching and assistance with medically necessary services such as laundry, cooking, cleaning, communication and social skills, recreation, grocery shopping, personal hygiene, and self-care skills.\\r\\n\\tMonitor medications in accordance with agency procedures to ensure that proper dosages are taken as prescribed.\\r\\n\\tAssist clients in the proper budgeting and use of their funds, acting as an advocate for clients to secure needed services and financial entitlements.\\r\\n\\tPerform on-call duty and weekend coverage on a rotating basis.\\r\\n\\tMaintain relevant documentation.\\r\\n\\tThis position is full-time (40 hours a week) working normal business hours.\\r\\n\\r\\n\\r\\nAbout Us:&nbsp; For 35 years, Community Support Services has been making a difference in the lives of countless individuals with severe and persistent mental illnesses.
