The successful candidate will be responsible for coordinating children and youth&rsquo;s medical and mental health care and providing children and youth with resources to foster continued good lifetime practices needed for independent living. The NCM encourages good health practices and preventative healthcare with foster parents and children/youth.</p><p style='margin:0in;font-size:15px;font-family:\"Times New Roman\",serif;margin-top:3.5pt;margin-right:64.1pt;margin-bottom:.0001pt;margin-left:0in;text-align:justify;'>This includes developing, implementing, and evaluating individualized patient care plans for children on their caseload.
