RN Clinical Manager – Hospice Homecare
Summary : The Hospice Clinical Manager oversees the daily operations of the clinical team, ensuring quality care through effective coordination and compliance with regulations. They manage staff performance, provide ongoing training, and foster a supportive work environment. This role involves collaborating with other managers to optimize staffing, control costs, and meet productivity targets. Additionally, the manager leads quality improvement initiatives to maintain high standards of patient care.
Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values. Provides a positive work environment by fostering open communication, addressing concerns, and ensuring a safe, efficient work environment. Monitors staff climate and ensures a culture of support and accountability. Responsible for daily business operations and clinical coordination of the interdisciplinary team in providing high quality home hospice care. Organizes and facilitates an efficient and effective Interdisciplinary Team Care Conference for achieving optimum clinical and financial agency outcomes and assuring the development, implementation, and updates of the individualized plan of care. Manages daily clinical operations, including the review of clinical documentation to ensure compliance with regulatory standards, processing of clinical workflow tasks, coordination of clinical visits, and review of physician orders. Initiates prompt follow-up with clinicians for any necessary documentation corrections. Assumes 24-hour accountability for the management and coordination of human and material resources, ensuring adequate on-call staffing coverage and effective communication to provide continuous, high-quality patient care services. Monitors staff performance and ensures accountability for personal, professional goals, and key performance indicators. Evaluates clinical competence and compliance, providing timely feedback and coaching as needed. Promotes continuous staff development through regular training and development activities, including monthly staff meetings. Utilizes staffing metrics for short- and long-range planning across the organization, collaborating with other clinical managers to ensure optimal staff utilization and productivity across all areas. Assists with budget development, manages cost containment, and ensures that productivity goals are met throughout the organization, not just within individual branches. Implements the quality assurance and performance improvement (QAPI) program across the department, fostering collaboration among all teams to align with organizational processes and standards, and working together to consistently deliver high-quality care. Performs all other duties as necessary.
Work Schedule: 80 Hours Bi-weekly. Full Time
Qualifications/Training/Licenses:
Minimum of 2 years of experience in home health or hospice. SC Nursing License Must be a graduate of an approved school of professional nursing and licensed in the state in which he / she currently practices. Must possess a current valid SC driver’s license and automobile insurance as required by law. Must be at least 21 years old to be covered under McLeod Health’s fleet vehicle insurance policy. License and registration as required by state law. Licensed registered nurse. BLS Certification. BLS (American Heart Association)
