Coordinates patient services. Assesses appropriateness of therapy, dispenses medications, supplies and medical equipment, documents patient progress toward goals, intervenes as clinically appropriate and provides education regarding appropriate medication administration and use. Additional dimensions may include working with operating budget, revenue responsibility, purchasing responsibility, etc.</p><p>Ensures company&rsquo;s goals and objectives are aligned with the organization&rsquo;s broader mission, vision, and driving strategies. Directly accountable and reports to the Pharmacy Manager or Director of Pharmacy or Operations based on branch leadership.</p><p><br><strong>MAJOR DUTIES AND RESPONSIBILITIES:</strong></p><p>1. Demonstrates ethical pharmacy practice and ensures compliance with all company policies and procedures, state and federal pharmacy rules and regulations, professional practice standards, and accrediting entities.</p><p>2. Assesses patient&#39;s suitability for home care and appropriateness of prescribed therapy.</p><p>3. Ensures the patient database is complete and compiles the necessary information needed to design a patient-specific pharmacy plan of care with goals for therapy.</p><p>4. Recommends and writes patient-appropriate orders.</p><p>5. Assures documents that require the prescriber&#39;s or pharmacist&#39;s signature for billing are generated and obtained.</p><p>6. Assesses laboratory results and performs pharmacokinetic dosing.</p><p>7. Makes appropriate dosing recommendations and documents.</p><p>8. Initiates, creates and updates patient medication profiles.</p><p>9. Performs accurate prescription order entry to ensure medications are properly concentrated, without incompatibilities, stable and properly stored.</p><p>10. Dispenses medications and related supplies to cost-effectively meet patient needs and assures authorization for services is obtained prior to dispensing.</p><p>11. Accurately dispenses, programs, if applicable, and troubleshoots medication administration pumps.</p><p>12. Accurately submits electronic claims for medications, as applicable.</p><p>13. Documents all communication with physicians, nurses, dietitians, patients and caregivers in the patient&#39;s medical record.</p><p data-pasted=\"true\">14. Documents initial patient or caregiver discussion, education and counseling regarding prescribed therapy, medication use and information, infusion method, and after hours contact information.</p><p>15. Thoroughly and objectively documents the patient&#39;s course of therapy, compliance with therapy, laboratory results, and progress towards goals of therapy with therapy-appropriate frequency.</p><p>16. Ensures all compounding, clean room and anteroom activities adhere to all state and federal regulations, USP &lt;797&gt; standards and company policies and procedures by all pharmacy staff.</p><p>17. Ensures required documentation is completed on QA/QC records and compounding documents.</p><p>18. Completes company-required orientation and annual competencies and in services.</p><p>19. Provides concise, applicable and timely responses to requests for drug information.</p><p>20. Accurately maintains calendar for laboratory tests, compounding, delivery, and documentation updates</p><p>21. Obtains and processes patient clinical information to allow for processing refills and supplies at least 24 hours in advance of patient&#39;s need.</p><p>22. Plans work to ensure cost-effective delivery methods are utilized.</p><p>23. Oversees the work of pharmacy support personnel and ensures their job duties are completed in a timely manner.</p><p>24. Delegates non-clinical tasks to assigned support personnel.</p><p>25. Prepares for and actively participates in patient care conferences including Nutrition Support Rounds.</p><p>26. Participates in the on-call process and documents on call activities in the patient medical record, if appropriate, and communicates with relevant personnel.</p><p>27. Assists in the training and supervision of pharmacy support personnel, pharmacy interns and pharmacy-profession students.</p><p>28. May assume role in of the Pharmacist in Charge or general branch manager during temporary absences and/or vacancies as appropriate.</p><p>29. Performs other related duties as directed by supervisor.<br><br></p><p data-pasted=\"true\"><strong>SKILLS AND ABILITIES:</strong></p><p>&bull; Excellent verbal and good written communication skills.</p><p>&bull; Advanced level of multi-tasking and problem-solving skills.</p><p>&bull; Moderate computer skills including MS Office, Windows Outlook, and Internet.</p><p>&bull; Moderate level of interpersonal skills to interact effectively and professionally with both internal and external customers.</p><p>&bull; Moderate mathematical skills to perform pharmaceutical calculations.</p><p>&bull; General knowledge of modern office procedures, filing, telephone techniques and office equipment.</p><p><br><strong>EDUCATION AND/OR EXPERIENCE:</strong></p><p>1. Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master&rsquo;s of Science degree in Pharmacy desirable) from an accredited college of pharmacy.</p><p>2. Currently licensed as a registered pharmacist in state of employment.</p><p>3. A minimum of two years recent experience in an acute-care setting or one year recent experience in home infusion therapy</p><p>4. Experience and strong knowledge base in intravenous admixture services and intravenous therapy.</p><p>5. Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals.</p><p>6. Must be able to work effectively with others to provide safe and effective drug therapy.</p><p>7. Must demonstrate ability to problem solve and supervise technical and other support staff.</p><p>8. Must be flexible with work schedule.</p><p>9. Good organizational and time-management skills.</p><p>10. Participation and membership in professional societies and organizations is desirable.</p><p>11. Excellent verbal and written communication skills.</p><p>12.
