This role designs and manages individualized treatment programs, counsels individuals within scope of practice, and helps address social, legal, and institutional adjustment needs. The position works closely with facility staff, probation, medical, behavioral health teams, and community agencies to ensure coordinated services. The Case Manager maintains accountability for services provided, supports continuity of care, and facilitates reentry-related services.\n<p> </p>\n<h2><strong>Key Responsibilities</strong></h2>\n<p> </p>\n<ul>\n <li>Design, develop, and coordinate individualized treatment programs to meet offender needs.</li>\n <li>Coordinate services with facility staff, community agencies, probation, medical, and behavioral health partners.</li>\n <li>Provide counseling and identify individual challenges to recommend appropriate solutions within scope of practice.</li>\n <li>Support continuity of care by arranging community appointments and reentry-related services.</li>\n <li>Maintain accurate documentation, comply with security and confidentiality requirements, and support classification and treatment processes.</li>\n</ul>\n<p> </p>\n<h2><strong>Additional Details</strong></h2>Requires current, unrestricted Nevada licensure.
