    "description": "<p><span style=\"font-weight: bold\">Position Summary:</span></p>\n<p><span style=\"font-weight: bold\">Case Manager </span></p>\n<p>The Case Manager plays a crucial role in coordinating and documenting client care to ensure seamless service delivery. They act as a liaison between healthcare providers, courts, families, and external agencies, facilitating comprehensive client support. Responsibilities include implementing case management services per individualized service plans, collaborating with the Treatment Team, and maintaining accurate documentation and reports. The Case Manager upholds strict client confidentiality, actively participates in team meetings, and attends required training. Additionally, they familiarize themselves with Medicaid billing processes and are prepared to assist in emergencies, including disaster evacuations. This position requires strong organizational, communication, and problem-solving skills to support clients effectively on their recovery and care journey.</p>\n<p> </p>\n<p> </p>\n<p><span style=\"font-size: 24pt\">GENERAL STANDARDS</span></p>\n<ul>\n<li>Is familiar with the New Hope Therapeutic Community model rules and regulations</li>\n<li>Completes responsibilities within designated timeframes.</li>\n<li>Meets residential program standards established by CARF, local, and government-related entities.</li>\n<li>Demonstrates knowledge of New Hope policies and procedures</li>\n<li>Addresses clients and other staff members appropriately</li>\n<li>Communicates client needs and concerns to the appropriate staff member, if necessary</li>\n</ul>\n<p><span style=\"font-weight: bold\">AGENCY REQUIRED STANDARDS</span></p>\n<ul>\n<li>Demonstrates awareness and sensitivity to all types of issues related to client diversity, including cultural, ethnic, language, gender preference, sexual orientation, physical handicap, and spiritual diversity.</li>\n<li>Keeps the work area clean and organized.</li>\n<li>Attends and actively participates in staff meetings at least 50% of the time.</li>\n<li>Able to increase work pace and prioritize accommodating emergencies or rush assignments.</li>\n<li>Clarifies assignments/responsibilities with the administrative staff.</li>\n</ul>\n<p> </p>\n<p><span style=\"font-weight: bold\">Essential Duties and</span></p>\n<p><span style=\"font-weight: bold\">Responsibilities as Case Manager:</span></p>\n<ul>\n<li>Assist with the coordination and documentation of client linkage to care.</li>\n<li>Act as liaison between health care providers, courts, families, and outside agencies.</li>\n<li>Run two to three groups related to Wellness Recovery Action Plan (WRAP) and Activities of Daily Living skills (ADL's)</li>\n<li>Provide case management services in accordance with the service plan as appropriate to the program.</li>\n<li>Please coordinate with the Treatment Team members and provide them with the appropriate reports before the meeting.</li>\n<li>Completes documentation and reports as necessary.</li>\n<li>Attends staff meetings weekly and participates as a team member.</li>\n<li>Upholds total confidentiality with respect to clients in all instances.</li>\n<li>Attend training and in-services as required.</li>\n<li>Performs other duties as appropriate and assigned.</li>\n<li>In case of an emergency, hurricane, or other disaster, the employee is willing to evacuate with the clients to the appropriate facility, if needed.</li>\n<li>Become familiar with the Medicaid billing process.</li>\n</ul>\n<p> </p>\n<p> </p>\n<p><span style=\"font-weight: bold\">Qualifications:           </span>To successfully perform in this position, the employee must be able to execute each duty satisfactorily.  The requirements listed below represent the knowledge, skills, and/or abilities required. <br>Reasonable accommodation will be provided to enable employees with disabilities to perform the essential functions.</p>\n<p><br><br></p>\n<p><span style=\"font-weight: bold\">Education and/</span></p>\n<p><span style=\"font-weight: bold\">or Experience:           </span>Bachelor’s degree in psychology, social work, or another related field is preferred.  One year of experience in counseling.  Must be able to provide their own vehicle, vehicle insurance, and have a current FL driver’s license.  In some instances, it may be necessary to transport clients in an agency vehicle; therefore, a class D license is preferred.  Experience working in an addiction counseling setting.</p>\n<p> </p>\n<p><span style=\"font-weight: bold\">Reasoning Ability:     </span>Ability to apply common sense understanding to carry out instructions furnished in written or oral form.</p>\n<p> </p>\n<p><span style=\"font-weight: bold\">Physical demands:</span></p>\n<p>The physical demands described here are representative of those required of an employee to perform the essential functions of this job successfully.
