This per diem role offers the opportunity to pick up hours based on facility needs, including weekdays, weekends, and holidays.</p>\n<p> </p>\n<p><strong>Position Overview:</strong>The PRN SLP will provide high-quality speech, language, cognitive, and swallowing therapy services to residents in a skilled nursing setting. This role requires adaptability, strong clinical judgment, and collaboration with an interdisciplinary team to deliver exceptional patient care.</p>\n<p> </p>\n<p><strong>Responsibilities:</strong></p>\n<ul>\n <li>Evaluate, treat, and document speech, language, cognitive, and swallowing disorders in accordance with physician orders and plan of care</li>\n <li>Develop and implement individualized treatment plans</li>\n <li>Maintain accurate, timely documentation in compliance with regulatory standards</li>\n <li>Collaborate with nursing, therapy team members, physicians, and families</li>\n <li>Participate in care planning and interdisciplinary team meetings</li>\n</ul>\n<p><strong>Qualifications:</strong></p>\n<ul>\n <li>Current New York State license as a Speech-Language Pathologist</li>\n <li>Certificate of Clinical Competence (CCC-SLP) preferred</li>\n <li>Experience in a skilled nursing facility preferred</li>\n <li>Strong clinical, organizational, and communication skills</li>\n <li>Ability to work independently and as part of a team</li>\n <li>Flexibility to work as needed based on facility demands</li>\n</ul>\n<p><strong>Why Join Elderwood?</strong></p>\n<ul>\n <li>Flexible scheduling</li>\n <li>Supportive and collaborative team environment</li>\n <li>Opportunity to work with a diverse geriatric population</li>\n</ul>\n<h2>Responsibilities</h2>\n<p><strong>Speech-Language Pathologist:</strong></p>\n<ul>\n <li>Screens resident for communication problem upon request; conducts an in-depth evaluation and makes written recommendations for a therapy plan of care, if necessary and upon request.</li>\n <li>Coordinates audiometric screening test for resident and makes referrals for further audiology evaluations, or treatments if necessary.</li>\n <li>Initiates speech-language therapy plan of care according to physician orders; and maintains required contact with the physician during the therapy program of resident.</li>\n <li>Observes rights of residents and treats residents with respect and kindness.</li>\n <li>Maintains written evaluations, progress notes, and discharge summaries for all residents in a speech-language treatment program.</li>\n <li>Confers with other staff regarding special needs of residents and integrates speech-language treatment program with goals and approaches of other disciplines.</li>\n <li>Attends Resident Care Planning and Family/Resident Care Plan Review meetings for residents currently in a speech-language treatment program.</li>\n <li>Maintains records that include statistical reports about resident, in-service education information, budget information and a material/equipment inventory.</li>\n <li>Submits statistical reports to the Business Office staff for Medicare billing payments, and completes assessment forms upon request.</li>\n <li>Attend staff meetings and required or preferred in-service training.</li>\n <li>Participates in developing and conducting in-service training programs for facility personnel involved in resident care.</li>\n <li>Reviews written speech-language policies and procedures annually and makes necessary revisions.</li>\n <li>Maintains costs of program within the parameters of the annual budget.</li>\n</ul>\n<h2>From</h2>\n<h2>Up to</h2>\n<h2>Qualifications</h2>\n<p><strong>Speech-Language Pathologist:</strong></p>\n<ul>\n <li>M.A. or M.S. Degree in Speech-Language Pathology from an approved school required.</li>\n <li>State license to practice as a Speech-Language Pathologist required.</li>\n <li>Clinical Competence Certification from the American Speech and Hearing Association preferred.</li>\n <li>Ability to communicate well verbally and in writing required.</li>\n <li>Current, valid CPR/BLS credential.</li>\n <li>Qualities of maturity, diplomacy, and ability to work well with others required.</li>\n <li>Verification of qualifications by the State Department of Health</li>\n <li>Rehabilitation Therapies (license number/certification number) required.</li>\n <li>Ability to perform tasks to established standards of excellence required.</li>\n <li>Demonstrated willingness to uphold the vision and mission of the company by following the core values of Integrity, Team work, Passion for Excellence and Reverence for the Individual is required.</li>\n <li>This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
