JOB STATEMENT/SUMMARY
The Coplin Health Systems (CHS) registered nurse care manager will facilitate preventive healthcare and improve overall patient outcomes by supporting clinicians through defined processes, workflows, and interventions, assuring continuity of care, and removing barriers to care.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES OF THE POSITION
Ensures new patient medical history is updated in patient charts and readily accessible for initial patient appointments with clinicians. Updates existing patient charts with relevant clinical data, testing, and procedures when identified through health information systems and networks. Coordinates with appropriate clinicians to facilitate preventative care needs of patients. Updates preventative care documentation required for payors and compliance purposes (e.g., UDS, HRSA). Coordinates, collects, and documents care and services information in collaboration with patient care teams as required for Patient Centered Medical Home. Facilitates communication with patients following hospitalizations and emergency department visits, collects associated clinical records, and uses the information to update charts in preparation for follow-up patient appointments within timeframes required by payors and proper management of patient conditions. Reviews patients that no-show for scheduled appointments and manages the no-show procedure in accordance with current policy. Communicates with eligible patients regarding medical documentation and appointments required for annual Medicare Wellness visits in accordance with established protocols and workflows. Prepares patient data from cooperating hospitals, health information networks, and care organizations, and reports in the daily clinical huddles at locations assigned to care manager. Assists clinical staff with preventative patient care coordination and documentation for WV Breast & Cervical Cancer Screening Program. Assists with the patient on-boarding and discharge process. Communicates directly with patient (and/or patient’s legal representative) regarding care and updates the patient record with appropriate information. Assists in identifying, procuring, and coordinating services and resources that to reduce barriers to patient care. Assists with community outreach opportunities to increase access to preventative care services. Assists uninsured and under-insured patients and community members to apply for insurance coverage through the Marketplace or other approved application services during annual enrollment periods and other appropriate times. Performs other duties as assigned by the Care Management Coordinator and/or Clinical Management with whom the care manager is collaborating to provide quality care to patients.

Candidates with active registered nursing licensure in West Virginia is required. Candidates with at least two years of nursing experience and active license in Ohio strongly preferred.
