The MDS Nurse Coordinator (RN) is responsible for the timely and accurate completion of Minimum Data Set (MDS) assessments in accordance with:</p>\n<ul>\n <li>RAI Guidelines,</li>\n <li>CMS regulations,</li>\n <li>OBRA requirements,</li>\n <li>Medicare and payer guidelines,</li>\n <li>and state/federal compliance standards.</li>\n</ul>\n<p>This role collaborates closely with Nursing, Therapy, Social Work, Dietary, Activities, Business Office, and facility leadership to ensure accurate assessment coding reflective of resident acuity and services provided.</p>\n<p> </p>\n<p>About Elderwood at Hamburg</p>\n<ul>\n <li>Elderwood at Hamburg is a respected skilled nursing and rehabilitation community committed to providing high-quality resident care through strong leadership, clinical collaboration, and compassionate service.</li>\n</ul>\n<p>Connect With Us Today. </p>\n<ul>\n <li>If you are an experienced MDS Registered Nurse looking for a supportive environment where your expertise is valued and your work makes a measurable impact - we encourage you to apply today.</li>\n</ul>\n<h2>Responsibilities</h2>\n<p>MDS NURSE (RN) Responsibilities Include:</p>\n<ul>\n <li>Completion and submission of MDS assessments for all payer types and assessment schedules</li>\n <li>Accurate coding of diagnoses, treatments, medications, functional status, and resident interviews</li>\n <li>Participation in interdisciplinary care planning and IDT meetings</li>\n <li>Collaboration with nursing and clinical departments regarding reimbursement and documentation needs</li>\n <li>Support regulatory compliance and survey readiness efforts</li>\n <li>Monitor assessment timelines and submission accuracy</li>\n <li>Communicate clinical reimbursement concerns appropriately</li>\n</ul>\n<h2>From</h2>\n<h2>Up to</h2>\n<h2>Qualifications</h2>\n<p><strong>MDS Nurse (RN) </strong>Qualifications:</p>\n<ul>\n <li>RN license in the state of New York. </li>\n <li>Prior MDS Coordinator / RNAC experience preferred</li>\n <li>Long-Term Care and Skilled Nursing experience preferred</li>\n <li>Experience with PointClickCare (PCC) preferred</li>\n <li>Knowledge of RAI process, Medicare, OBRA, and reimbursement guidelines</li>\n <li>Strong organizational and communication skills</li>\n <li>This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
