Acts as case manager and liaison for children, foster families, and those committed to helping youth in foster care reach their full potential.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\"><strong><u>Essential Duties:</u></strong><u>&nbsp;</u></span></p><ol><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Ensures that the self-expression, self-worth, and well-being of youth in PPCH Foster Care is supported and respected. &nbsp;<ul style=\"font-family: initial; font-size: initial;\"><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Promotes collaboration with youth in Foster Care, their loved ones, Foster Families, regulatory agencies, and support team.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Ensures that training, resources, and assistance are provided to Foster Families and others so they may create a safe and supportive home for youth in Foster Care.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Collaborates and attends meetings with foster families, schools, treatment teams, counties, and others as a representative of PPCH Foster Care.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Continually evaluates and assesses the programs&rsquo; strengths and weaknesses to improve services and supports using a person-centered approach.&nbsp;</li></ul></li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Supports a culture where children and others in services are healthy, safe, and valued by others.&nbsp;<ul style=\"font-family: initial; font-size: initial;\"><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Models and teaches respect for others, including youth in Foster Care, Foster Families, guardians and loved ones, and team members.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Ensures the safety and well-being of youth in PPCH Foster Care by providing oversight, guidance, and resources to Foster Families.</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Monitors medical services, appointment attendance and follow-up for youth in PPCH Foster Care. &nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Ensures that the preferences and needs of youth in PPCH Foster Care are considered.</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Conducts required monitoring, environmental studies, and home visits and ensures prompt follow-up when deficiencies or areas for improvement are found.&nbsp;</li></ul></li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Responsibility for compliance with the Colorado Rules and Regulations for Child Placement Agencies, PPCH policies, and the rules and regulations of other oversight agencies. &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;<ul style=\"font-family: initial; font-size: initial;\"><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Oversight of and responsibility for foster care tracking and documentation, such as ensuring bookkeeping, reporting, auditing, and recordkeeping meet the standards of PPCH and State and Federal agencies.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Ensures foster families and providers have completed required trainings, including annual refreshers, according to established timelines.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Maintains knowledge and understanding of PPCH and regulating agencies&rsquo; policies, procedures, rules, regulations, and guidelines.&nbsp;</li></ul></li></ol><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Must possess either a Bachelor&rsquo;s degree in the social or behavioral sciences, <u>OR</u></li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Bachelor&rsquo;s degree in a non-related field plus 2 years&rsquo; experience supervised by an individual with a master&rsquo;s degree in social or behavioral sciences. &nbsp;&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Successful completion of all required PPCH training</li></ul><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\"><strong>Experience:&nbsp;</strong></span></p><ul><li style=\"font-family: arial, sans-serif; font-size: 14px;\">2 years of experience in social services or related field is preferred. &nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">2 years of experience working with individuals with disabilities is preferred.&nbsp;</li></ul><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\"><strong>Knowledge, Skills, and Abilities:&nbsp;</strong></span></p><ul><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Excellent written and verbal communication skills, with effective listening skills, even when non-traditional communication is used.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Ability to collaborate with or manage others.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Strong organizational and time management skills.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Ability to respectfully address significant issues of health and safety with foster parents and teams.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Knowledge of foster care in Colorado and Volume 7 of Colorado State Rules and Regulations preferred. &nbsp;&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Skill and ability to constructively receive and provide feedback &ndash; Able to have difficult conversations.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Valid driver&rsquo;s license and reliable transportation with the ability to meet PPCH&rsquo;s driving requirements.&nbsp;</li></ul><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\"><strong><u>Physical Requirements and Working Conditions:</u></strong></span></p><ul><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Lifting and carrying of approximately 15 lbs and may be required to bend, stoop, squat as it pertains to assessing an environment.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Work is mainly performed in private homes or in the community using a laptop and/or mobile device and is seldom performed in a typical office environment.&nbsp;</li><li style=\"font-family: arial, sans-serif; font-size: 14px;\">Regular and frequent driving throughout the Pueblo and Colorado Springs areas is required. &nbsp;Occasional travel to the main PPCH office in Lakewood may be required.</li><li style=\"font-family: arial, sans-serif;
