Program Manager – Community Residential Settings
If you enjoy supervising a team of staff, assisting others in living as independently as possible while ensuring integrated services and supports for persons served are properly carried out, and meeting the Designated Coordinator 245D Qualifications; then this job was made for you!
Job Purpose: The Program Manager, CRS, is responsible for oversight of the location(s) assigned and provides assistance, support, and encouragement to meet the needs of people with disabilities served by HMC. Functions as the designated coordinator and/or the QIDP. They are responsible for carrying out the directives of the individual’s team. Services are provided in the individual’s homes and in the community.
We are seeking motivated and dedicated superstars to join our team!
Schedule: Hours are primarily Monday – Friday from 8:00 am – 4:30 pm but will require some alternative hours for meetings and/or to meet the needs of the individuals served. Program Managers may need to work evenings and weekends as needed to help with direct care coverage. This position includes on-call.
Wage: $54,080 to $59,280 Annual Salary ($26.00 to $28.50 per hour depending on experience)

Provide assistance and support to individuals served with developmental or intellectual disabilities. Take leadership in advocating for the benefit of others. What do we need from you? Must be 18 years of age or older CRS Program Manager must meet the criteria of a Designated Coordinator as per MN Statute 245D. Prefer a bachelor’s degree in a human service-related field and at least one year of experience working directly with individuals with intellectual or developmental disabilities.
The designated coordinator must minimally have: (1) a baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; (2) an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; (3) a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or (4) a minimum of 50 hours of education and training related to human services and disabilities; and (5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3). Experience as a leader/supervisor of staff, particularly in the healthcare field. Must pass the MN Department of Human Services Background Study Must pass a driving record check conducted by our insurance carrier, with valid US driver's license Willingness to learn
