Working in a brand new, 12 bed state-of-the-art facility, Bluestone staff will have a unique ability to direct patient care, employ best practices, and join a care network that promotes the best long-term outcomes for patients.</p><p><strong>POSITION SUMMARY:</strong> Bluestone Child & Psychiatric Hospital is hiring Social Workers to be a part of our new facility! As the Clinical Social Worker, you will be responsible for providing social service functions in a behavioral healthcare setting. You will provide assessments as well as treatments and case management to patients and families. You will also be a resource person to healthcare professionals concerning psycho-social problems, community resources.</p><p><b>RESPONSIBILITIES INCLUDE:</b></p><ul><li>Demonstrate good theoretical knowledge of psychopathology and appropriate treatment techniques for each type of therapy.</li><li>Perform psychosocial assessments on all patients to determine social service needs, diagnosis and plan of care.</li><li>Evaluate patient data, develops and implement a plan of care for the patient.</li><li>Communicate with physicians regarding social service needs of the patient. Assist the physician to facilitate inpatient care.</li><li>Participate in plan of care for each patient; work cooperatively as a member of the interdisciplinary treatment team; completes progress notes and discharge plans in a timely fashion.</li><li>Provide individual patient and family therapy and crisis intervention.</li><li>Provide care appropriate to condition for pediatric or adolescent population.</li><li>Observe and communicate pertinent information regarding patient mental status and behavior, occurrences, and/or negative response to treatment or intervention to physician and/or appropriate health care team members.</li><li>Engage in therapeutic relationships that focus on patient needs and provide a structured process for patients to reach objectives outlined in the plan of care.</li><li>Maintain current knowledge of social, medical and psychiatric service available within the community; maintains supply of resource materials to be distributed to patients when needed.
