    "description": "<p><span style=\"font-weight: bold\">Position Summary:</span></p>\n<p>Responsible for following Bridges to Wellness program policies and protocols to help clients</p>\n<p>and their families navigate and access community services, other resources, and adopt healthy</p>\n<p>behaviors.  Will be expected to complete additional trainings, such as Cribs for Kids, to be able to</p>\n<p>add that knowledge for service delivery to families.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Essential Duties:  </span><em><span style=\"font-size: 10pt\">To perform this job successfully, an individual must be able to satisfactorily perform each essential duty listed below.</span></em><em><span style=\"font-size: 10pt\"> </span></em></p>\n<ul>\n<li><span style=\"font-size: 12pt\">Contribute to creation and completion of specific performance objectives in personal work plan which links to the ZMCHD strategic plan.</span></li>\n<li>Establish positive, trusting relationships with clients and their families while providing general support, encouragement, and feedback.</li>\n<li>Motivate clients to be active, engaged participants in their health.</li>\n<li>Remain knowledgeable of community resources appropriate for needs of clients. Actively assist with referral process for agency and community resources and programs.</li>\n<li>Follow-up with clients via phone calls, home visits, and visits to other settings where clients can be found. Follow up should be continuous from initial eligibility through closure of need.</li>\n<li>Assist clients with accessing health related services, completing applications and forms, understanding care plans and instructions, utilizing services</li>\n<li>Give appointment reminders and attend appointments (as approved)</li>\n<li>Help connect clients with transportation resources. (Transporting clients is strictly prohibited without prior permission)</li>\n<li>Work closely with internal care team to help ensure clients have comprehensive and coordinated care.</li>\n<li>Act as a patient advocate and liaison between client/family and community service agencies.</li>\n<li>Enter data into electronic and hard copy records, spreadsheets, and databases. Complete client reporting within two business days.</li>\n<li>Participate on committees, meetings, trainings, and events as requested.</li>\n<li>Maintain confidentiality of confidential or sensitive records and information pertinent to the assigned position.</li>\n</ul>\n<p><span style=\"font-weight: bold\">Qualifications: <span> </span></span><em><span style=\"font-size: 10pt\">An appropriate combination of educati<span>on, training, course work and experience may qualify an applicant to demonstrate required knowledge, skills, and abilities. An example of an acceptable qualification is:</span></span></em></p>\n<ul>\n<li><span>High school diploma or GED</span></li>\n<li>Successful completion of a Community Health Worker formal training program within 12-month period that results in licensure as a Certified Community Health Worker</li>\n<li>Knowledge of community resources and medical terminology necessary</li>\n<li>Experience working with diverse populations in a community-based setting preferred</li>\n<li>Basic computer knowledge with competency in application of Microsoft Word, Excel, and other job-related computer software</li>\n<li>Ability to demonstrate flexibility and learn administrative, clerical and other responsibilities specific to the job assignment</li>\n<li>Valid driver’s license, good driving record, and access to reliable transportation</li>\n<li>Non-tobacco user</li>\n</ul>\n<p><em><br></em></p>\n<p><em><span>Ability to document identity and employment eligibility within 3 days of original appointment as a condition of employment in compliance with Immigration Reform &amp; Control Act requirements.</span></em></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Physical Demands:<span> </span></span><em><span style=\"font-size: 10pt\">Reasonable accommodations will be made for disabled persons, covered by the Americans with Disabilities Act, in accordance with its requirements.</span></em></p>\n<ul>\n<li><span>Regularly drives a motor vehicle to perform duties of job including but not limited to home visits, schools, presentations and meetings</span></li>\n<li><span>Frequently sits for extended periods of time</span></li>\n<li><span>Regularly stands, walks, and climbs stairs.</span></li>\n<li><span>Regularly exhibits manual dexterity when working on the computer, typing, entering data and performing other related tasks.</span></li>\n<li><span>Vision demands include close, relatively detailed vision, with the ability to adjust focus when typing or reading a computer screen.</span></li>\n<li><span>Regularly talks and hears when working with the general public.</span></li>\n<li><span>Occasionally lifts up to 25 pounds.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Work Environment:</span></p>\n<ul>\n<li>General office setting in the health department facility; may be able to work partly remotely</li>\n<li>Exposure to home, school, and other community settings and environments</li>\n<li>Exposure to heat, cold, snow, ice, and other inclement conditions</li>\n<li>Employee must use standard safety and health precautions when exposed to bloodborne pathogens</li>\n</ul>\n<p><br></p>\n<p><em><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\"><span style=\"font-size: 10pt\">This job description in no manner states or implies that these are the only duties and responsibilities to be performed by the employee filling this position, who will be required to follow  instructions and perform any duties required by the employee's supervisor or designee.</span></span></span></em></p>",
