This position works <strong>directly with Interventional Cardiologists</strong> to support the delivery of multidisciplinary care for patients with complex cardiovascular and structural heart disease, and includes close care coordination and relationship-building with <strong>Advanced Heart Failure (AHF)</strong> and <strong>Cardiothoracic Surgery (CTS)</strong> teams.</p>\n<p>This role is ideal for a Nurse Practitioner with a strong cardiology background who thrives in a fast-paced, highly specialized environment and is committed to delivering coordinated, patient-centered cardiovascular care.</p>\n<p><strong>About the Opportunity</strong></p>\n<p>The Interventional Cardiology Nurse Practitioner plays a key role in managing patients across the continuum of care, including <strong>outpatient clinic visits, inpatient evaluations, procedural planning, and post-intervention follow-up</strong>. The NP partners closely with Interventional Cardiologists to support clinical care delivery while ensuring seamless coordination with Advanced Heart Failure and Cardiothoracic Surgery services as needed.</p>\n<p>While invasive procedures are performed by Interventional Cardiologists, the NP is instrumental in optimizing patient preparation, education, and post-procedural management, helping ensure high-quality outcomes and a superior patient experience.</p>\n<p> </p>\n<p><strong>Key Responsibilities:</strong></p>\n<h3>Clinical Care</h3>\n<ul>\n <li>Provide advanced clinical care to cardiac patients in <strong>both outpatient and inpatient settings</strong>, including new patient evaluations, follow-up visits, and hospital-based consults</li>\n <li>Perform comprehensive cardiovascular assessments, order and interpret diagnostic studies, and develop treatment plans in collaboration with Interventional Cardiologists</li>\n <li>Support <strong>pre- and post-procedural care</strong> for interventional and structural heart procedures</li>\n</ul>\n<h3>Care Coordination & Multidisciplinary Collaboration</h3>\n<ul>\n <li>Work directly with <strong>Interventional Cardiologists</strong> to support day-to-day clinical operations and patient management</li>\n <li>Coordinate care with <strong>Advanced Heart Failure and Cardiothoracic Surgery</strong> teams to ensure timely, seamless multidisciplinary care</li>\n <li>Participate in <strong>multidisciplinary heart team and valve conferences</strong>, contributing to patient selection and care planning</li>\n <li>Support transitions of care, including hospital discharge planning and outpatient follow-up</li>\n</ul>\n<h3>Patient Education & Communication</h3>\n<ul>\n <li>Educate patients and families on cardiovascular conditions, recommended interventions, procedural expectations, and recovery pathways</li>\n <li>Conduct <strong>in-person and telehealth visits</strong>, and manage patient communication through the <strong>Epic electronic health record (EHR)</strong></li>\n <li>Serve as a clinical resource for patient questions, symptom management, and follow-up coordination</li>\n</ul>\n<h3>Program Support & Quality Initiatives</h3>\n<ul>\n <li>Support quality improvement, patient safety, and patient experience initiatives within Interventional Cardiology</li>\n <li>Assist with clinical research activities, registry participation, and program development initiatives as applicable</li>\n</ul>\n<h2>Conditions & Interventional Focus</h2>\n<p>The Nurse Practitioner will support patients with complex cardiovascular and structural heart disease, including:</p>\n<ul>\n <li>Structural heart disease</li>\n <li>Aortic, Mitral, and Tricuspid valve disease</li>\n <li>Advanced heart failure requiring interventional therapies</li>\n <li>High-acuity cardiology patients managed by Interventional Cardiology</li>\n</ul>\n<p> </p>\n<h2>Qualifications</h2>\n<h3>Education</h3>\n<ul>\n <li>Graduate of an <strong>accredited Nurse Practitioner program</strong> (MSN or DNP)</li>\n</ul>\n<h3>Licensure & Certifications</h3>\n<ul>\n <li>Current <strong>California Nurse Practitioner license</strong></li>\n <li>DEA Controlled Substance Registration Certificate (CSRC)</li>\n <li><strong>Basic Life Support (BLS)</strong> certification</li>\n <li><strong>Advanced Cardiac Life Support (ACLS)</strong> certification</li>\n <li>Additional certifications as required by the department</li>\n</ul>\n<h3>Experience</h3>\n<ul>\n <li><strong>Prior experience in cardiology is required</strong></li>\n <li>Experience in <strong>interventional cardiology, structural heart disease, heart failure, or inpatient cardiology strongly preferred</strong></li>\n <li><strong>New graduate Nurse Practitioners will not be considered</strong></li>\n <li>Demonstrated ability to care for complex cardiac patients and work effectively in a multidisciplinary clinical environment</li>\n</ul>\n<p> </p>\n<h2>Join Us and Enjoy</h2>\n<h2>Why Join SWBMG</h2>\n<ul>\n <li>Work closely with a <strong>high-performing team of specialized Interventional Cardiologists</strong></li>\n <li>Be part of an innovative, growing cardiovascular program serving the San Francisco community</li>\n <li>Practice in a collaborative, team-oriented environment that values clinical expertise and professional partnership</li>\n <li>Opportunities to contribute to <strong>clinical research and program development</strong></li>\n <li>Competitive compensation and a comprehensive benefits package</li>\n <li>Strong support for continuing education and professional development</li>\n</ul>\n<h2>Organization Details</h2>\n<p>Sutter West Bay Medical Group is a premier multi-specialty medical group made up of over 400 physicians and advanced practice clinicians who care for patients at several San Francisco Bay Area locations.
