    "requisitionDescription": "<div><div><p><strong>POSITION PURPOSE:</strong><br><br>Responsible for delivering Healthy Steps services and coordinating the efforts of all the team to implement the Healthy Steps program and services.<br><br><strong>POSITION REQUIREMENTS / QUALIFICATIONS:</strong><br><br><strong><ins>Education/Experience</ins></strong>:&nbsp;&nbsp;&nbsp;&nbsp;<br>Bachelors in Health Science related field required with focus on Child Development specifically age 0-5. Experience working with children and families required. Previous exposure or experience to Healthy Steps program preferred.<br>&nbsp;&nbsp;&nbsp;&nbsp;<br><strong><ins>Licensure / Certification:</ins></strong> &nbsp;&nbsp;&nbsp;<br>Maintain current CPR certification from the American Heart Association<br>&nbsp;&nbsp;&nbsp;&nbsp;<br><strong><ins>Skills / Ability:</ins></strong> &nbsp;&nbsp;&nbsp;<br>Bi-lingual (English/Spanish) preferred, satisfactory knowledge of Child Development 0-5 required.<br><br><br><strong>POSITION RESPONSIBILITIES (THIS IS A NON-EXEMPT POSITION)</strong><br><br>Recruits and enroll families.<br><br>Meets with families at every well child visit, either with the physician/pediatric nurse practitioner or separately.<br><br>Conducts waiting room activities, as appropriate, during regularly scheduled office times.<br><br>Conducts home visits at recommended intervals.<br><br>Assesses child&rsquo;s developmental progress.<br><br>Prevents/intervenes/manages child development and common behavior problems.<br><br>Identifies quality resources and provide referrals and follow-up as appropriate to foster community connections and develop a working relationship with the contact person at referral agencies.<br><br>Facilitates/leads parent groups and support groups.<br><br>Documents all home visits and well care baby visits accurately and keeps documentation current.<br><br>Provides educational materials to family as scheduled and as needed.<br><br>Inputs information in SAMIS system and Credible EHR for all services rendered as services are rendered.<br><br>Administers and scores all screenings and maintains scores in SAMIS system and Credible EHR .<br><br>Ensure participant demographic and contact information is accurate.<br><br>Follows &ndash;up with families regarding questions within at least 2 days and with referrals within at least 7 days with assistance of Developmental Specialist.<br><br>Provides referrals to all families as scheduled and as needed based on screening scores and concerns of pediatrician.<br><br>Assist with follow-up of client compliance.<br><br>Demonstrates flexibility and change oriented.<br><br>Complies with CHI&rsquo;s Policies and Procedures at all times.<br><br>Adheres to Confidentiality Policies and Procedures / HIPAA Regulations.<br><br>Completes mandatory in-service trainings.<br><br>Communicates and solves problems through the proper chain of command.<br><br>Provides services in a sensitivity non-judgmental and non-discriminatory manner to a diverse population.<br><br>Attends required staff meetings.<br><br>Maintains flexibility in regard to expected or unexpected changes in the work environment.<br><br>Maintains compliance with regulatory entities and gran requirements.<br><br>Completes B&amp;E Forms within the same day of intervention if necessary.<br><br>Performs other duties as assigned.&nbsp;&nbsp;&nbsp;&nbsp;<br><br></p></div></div>\n",
