This role leads care management initiatives focused on improving patient outcomes, advancing population health, and reducing healthcare costs through data analytics, policy development, quality improvement, and cross-department collaboration. The Supervisor supports high-risk patients by ensuring coordinated, comprehensive care across healthcare settings and promoting best practices to improve continuity of care and close healthcare gaps. Job Qualifications: High School Diploma or GED Equivalent required Associate degree or higher in a relevant subject area such as public health, nursing, health administration, social work, business administration preferred; equivalent combination of education and relevant work experience may be considered. 2 to 5 years’ experience in population health and underserved populations.
