Description Aviva Family and Children’s Services Mission Statement The mission of Aviva Family and Children’s Services is to provide a comprehensive range of diverse and culturally sensitive therapeutic and educational programs to help children at-risk and emotionally distressed, abused, and neglected youth—as well as their families—function more effectively as individuals and within the family and community Job Summary Under the supervision of the Wraparound Program Lead, the Wraparound Parent Partner works closely with the Wraparound child’s parents/caregiver to represent their best interests and participate as a member of the Child and Family Team. The Parent Partner provides in-home support services as outlined in the Plan of Care. The Parent Partner completes all required paperwork in accordance with agency and county standards. Essential Functions o Responsiveness to the families’ identified needs. o Building and strengthening the child and family’s support system. o Development of Family-centered help-giver skills. o Engaging and creating of professional and Family partnerships. o Incorporation of informal supports, natural Family helpers, and linkage other Community resources. o Demonstrates creativity and resourcefulness in order to effectively navigate systems of care. o Ability to connect with families of various backgrounds and engage caregivers so they are open to teaming with Wraparound. o Shares the story of own journey in order to connect, empower, and give home to parents. o Adheres to a non-judgmental, strength-based model that builds on strengths and hope, while also being realistic and practical. o Outcomes for children and Families o Feedback from children and Families Community services providers. o Ability to build protective capacity with families; and Core Practice Indicators (engagement, teaming, assessment and understanding, long term view, planning, cultural competence, transitions, crisis management). o Work closely and maintain weekly visit with parents and caregivers to provide intensive and supportive services needed o Link and advocate for resources in the school setting and community to address the family’s needs identified in the family’s plan o Assist parents in identifying needs, barriers and problem solves to assist in the implementation of the family’s plan o Collaborate and teaming with other professionals to plan and coordinate immediate services to stabilize sudden crisis o Document all services provided following the Wraparound and DCFS standards (i.e. DMH billing, contact notes, SIR’s, Flex funds, CFT minutes, Authorization forms, etc.). o Meet for individual meetings with both the out-of-home caregiver and parent/current caregiver. o Participates in weekly Child and Family Team (CFT) meetings o Works collaboratively and cooperatively within a multidisciplinary team o Consult on regular basis with supervisor and team members about pertinent issues regarding the family’s participation and treatment plan o Assist parents/caregivers in keeping appointments, access and link parents/caregivers to court hearings, and other community or county programs. o Available 24/7, responsible for responding and supporting parents/caregivers during crisis calls o Sensitive and responsive to the cultural differences among the agency’s client population o Participation in all agency required trainings which includes but is not limited to Wraparound Basic Training, Trauma Informed Care and Elements of Wraparound o Actively participate in parent support groups/activities and quality improvement projects o Adheres to all applicable Federal and State regulations for confidentiality, HIPAA, mandated reporting of suspected child or elder abuse o Actively participate in quality improvement projects The Parent Partner will take on special projects or additional duties as assigned by the Wraparound Program Lead and/or Wraparound Coordinator. Qualifications Competency Requirements -Ability to speak, read, write, and understand English -Ability to work effectively as a member of the Wraparound team -Ability to successfully work with families of diverse ethnic and cultural backgrounds -Must have excellent oral and written communication skills -Must have excellent interpersonal skills -Knowledge of social services system and community resources -Knowledgeable in Wraparound philosophy, policy and process -Skilled in relationship building, conflict mediation and facilitation a plus -Basic computer knowledge (windows, Microsoft word, etc.) -Bilingual (Spanish) preferred Education and Experience -High School diploma or equivalent required -Parent Partner must have been the parent or primary caregiver (Primary caregiver is defined as a parent (not foster parent) who has 24/7 legal responsibility for the child/youth who received services from one of the following County departments: Department of Mental Health, Probation Department, and/or Department of Child and Family services; or from Regional Center. -At the time of hire, Parent Partners must not have an open probation case nor DCFS for at least one (1) year. -A Parent Partner can have a child who receives or received services from the State Regional Center or DMH. Certifications/Licenses -Valid CA driver’s license -Valid automobile insurance Additional Requirements -Tuberculosis clearance -Department of Justice/Fingerprint clearance -Satisfactory completion of health screening -Proof of legal eligibility to work in the United States -Satisfactory driving record Delegates Authority To N/A Physical Demands While performing the duties of a Parent Partner, the employee is frequently required to walk, drive, and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to sit and stand. The employee is rarely required to run, bend, kneel or lift objects weighing more than 30 pounds. Work Environment The work environment is indoor and outdoors. The noise level varies from quiet to loud. The employee will conduct home visits.
