If you want to feel honor and pride in the work that you do, while supporting a meaningful mission, come join our HopeWest Team!</em></strong></p><p>The Physical Therapist is responsible for:</p><ul type=\"disc\"><li>Performing comprehensive physical therapy assessments for new and existing participants.</li><li>Developing individualized, realistic and goal oriented plan of care (POC) for each participant.</li><li>Participating in Interdisciplinary Team (IDT) meetings daily, and plan-of-care meetings as assigned.</li><li>Providing appropriate physical therapy interventions according to the POC.</li><li>Educating the participants, family members, and/or caregivers about exercises and special techniques.</li></ul><p><strong>What is PACE?</strong></p><p>PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors&#39; health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.</p><p>The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.</p><p><strong>You Bring:</strong></p><ul type=\"disc\"><li data-pasted=\"true\">Graduate from a CAPTE accredited Physical Therapy program.</li><li>Currently licensed as a PT by the state of Colorado and maintained throughout employment.</li><li>1 year of PT experience required; 2 to 3 years of home health or other home-based PT experience preferred.</li><li>Valid Colorado driver&rsquo;s license, unlimited access to reliable transportation, and adequate automobile insurance is maintained throughout employment.</li><li>Able to work in a variety of environments.</li><li data-pasted=\"true\">Adequate computer literacy in MS Word, Excel, Access, Outlook&nbsp;</li><li>Current CPR certification is maintained throughout employment.&nbsp;</li><li>Excellent interpersonal and communication skills.</li><li>Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.</li><li>Effectively work with and build relationships with others.</li></ul><p><strong>We Offer:</strong></p><ul type=\"disc\"><li>Competitive pay structure starting at $43.38-$48.00 per hour, depending on experience.</li><li>UMR Health, Delta Dental, and VSP Vision insurance effective the month following start date.</li><li>Immediately start accruing PTO and medical leave.</li><li>Colorado Family and Medical Leave Insurance (FAMLI) program.</li><li>15% discount at Spoons Bistro &amp; Bakery, and Heirlooms locations.</li><li>Up to 3% match on 403B retirement.</li><li>Mileage reimbursement for work related driving at 65 cents per mile.</li><li>Employer paid life insurance and Employee Assistance Program.</li><li>Tuition reimbursement program</li></ul><p><strong>Together, through creativity, volunteerism, and philanthropy we change the experiences of aging, illness, and grief &ndash; one family at a time.</strong></p><p><em>HopeWest honors our service members and encourages veterans to apply.</em></p><p><em>This position will be open until filled.&nbsp;</em></p></div></div></div></div></div></div></div></div></div>\n",
