You will work under the direction and supervision of a licensed Physical Therapist to implement personalized treatment plans that help patients recover from injury, surgery, or illness and regain independence in their daily lives.</p><p>Key Responsibilities</p><p><br></p><p>Provide skilled physical therapy interventions in patients&rsquo; homes as outlined in the PT&rsquo;s plan of care</p><p>Assist patients with therapeutic exercises, functional mobility training, gait training, transfer training, and use of assistive devices</p><p>Perform manual therapy techniques, modalities (ultrasound, e-stim, etc.), and therapeutic activities</p><p>Educate patients and caregivers on home exercise programs, safety, and fall prevention</p><p>Document progress notes accurately and timely in our EMR system (experience with Homecare Homebase or similar preferred)</p><p>Communicate effectively with the supervising PT, case managers, and other members of the interdisciplinary team</p><p>Manage a productive caseload while maintaining compliance with Medicare and state regulations</p><p><br></p><p>Qualifications</p><p><br></p><p>Graduate of an accredited Physical Therapist Assistant program</p><p>Current PTA license in [State] (or eligible for immediate licensure)</p><p>Valid driver&rsquo;s license, reliable transportation, and auto insurance</p><p>Current CPR/BLS certification</p><p>Minimum 1 year of clinical experience preferred (new grads with strong home-health interest will be considered)</p><p>Home health experience is a plus but not required &ndash; we provide training and mentorship</p><p>Excellent time-management and documentation skills</p><p>Compassionate, patient-centered approach with the ability to work independently</p><p><br></p><p>What We Offer</p><p><br></p><p>Competitive pay</p><p>Flexible scheduling (full-time, part-time, or per diem)</p><p>Mileage reimbursement</p><p>Health, dental, and vision insurance (for full-time employees)</p><p>401(k) with company match</p><p>Paid time off and holidays</p><p>Continuing education reimbursement</p><p>Supportive clinical team, weekly case conferences, and direct access to supervising PTs</p><p>Laptop/tablet and supplies provided</p><p><br></p><p>If you&rsquo;re a motivated PTA who loves building relationships with patients in their own homes and wants the flexibility and autonomy that home health offers, we&rsquo;d love to meet you!</p></div></div></div>\n",
