We compassionately provide a continuum of services ranging from crisis housing to permanent housing and homelessness prevention. We seek employees who are passionate about serving and making a difference in the lives of others.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Summary</span></p>\n<p>The Housing Stabilizer supports families (ages 18–30) in maintaining long-term housing stability after exiting homelessness. This role focuses on barrier reduction, housing retention, life skills support, and landlord/tenant relationship strengthening.</p>\n<p>Participants are often dual-enrolled in UBH’s Rapid Rehousing (RRH) programs, so the Housing Stabilizer works closely with RRH Case Managers, Employment Specialists, and Mental Health Clinicians to provide coordinated, non-duplicated services.</p>\n<p>This program is supported by private funders and requires strong documentation, home visiting, and care coordination.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">ESSENTIAL FUNCTIONS</span></p>\n<p>Housing Stability &amp; Support</p>\n<p>· Conduct home visits and weekly follow-ups to support participants with maintaining housing.</p>\n<p>· Provide stabilization-focused services (not clinical case management): coaching, problem-solving, budgeting support, childcare navigation, and resource linkage.</p>\n<p>· Develop and update Housing Stabilization Plans with participants.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Landlord &amp; Tenancy Support</span></p>\n<p>· Serve as a liaison between participants and landlords to address concerns early.</p>\n<p>· Troubleshoot habitability issues, neighbor conflicts, rent challenges, and other tenancy risks.</p>\n<p>· Educate landlords about UBH support, communication protocols, and available resources.</p>\n<p>Cross-Program Collaboration</p>\n<p>· Participate in case conferences with RRH, Mental Health, and Employment teams for dual-enrolled families.</p>\n<p>· Coordinate services and ensure goals and interventions are aligned across programs.</p>\n<p>Data, Documentation &amp; Assessments</p>\n<p>· Maintain accurate records of home visits, progress notes, and landlord interactions.</p>\n<p>· Complete funder-required assessments, including the Family Resources Scale every six months.</p>\n<p>· Support reporting for private donor requirements.</p>\n<p>Resource Support &amp; Economic Mobility</p>\n<p>· Connect participants to childcare, healthcare, transportation, community resources, and financial supports.</p>\n<p>· Partner with Employment/Economic Empowerment staff to support income growth and work readiness.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">QUALIFICATIONS</span></p>\n<p>· BA in Social Work, Human Services, or related field or equivalent experience.</p>\n<p>· Experience working with TAY (18–30), families, or formerly homeless populations.</p>\n<p>· Ability to conduct home visits and provide field-based services.</p>\n<p>· Strong communication, organization, and problem-solving skills.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">MANDATORY REQUIREMENTS</span></p>\n<p>1) Employment Eligibility Verification</p>\n<p>2) Reliable transportation</p>\n<p>3) Updated tuberculosis test</p>\n<p>4) Successful completion of background screening.</p>\n<p>5) CPR/First Aid training</p>\n<p>6) Must have Valid CA Driver's License</p>\n<p>7) Must provide proof of insurance coverage</p>\n<p>8) Must be able to qualify for UBH drivers’ insurance coverage</p>\n<p><br></p>\n<p>PHYSICAL DEMANDS, ENVIRONMENTAL CONDITIONS, EQUIPMENT</p>\n<p><br></p>\n<p>The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
