In this position, you’ll be responsible for supporting patients and families in accessing community programs and resources to promote healthy outcomes and well-being. The Senior Community Health Worker is a vital part of the care and will assist patients and families with applications for public benefits and community programs; educate families on healthy behaviors and community resources; and be an advocate for the patient and family.</p><p>&nbsp;</p><p><i>Think you’ve got what it takes?</i></p><p>&nbsp;</p><p>&nbsp;</p><p><strong>Job Duties &amp; Responsibilities</strong></p><p>Screens patients and families for social drivers of health&nbsp;<br>Assists families in identifying and meeting goals to promote health and well-being&nbsp;<br>Identifies appropriate community resources and submits referrals for families&nbsp;<br>Provides education to families on community resources, healthy behaviors, and healthcare navigation&nbsp;<br>Assists families in accessing community programs and medical services&nbsp;<br>Assists patients and families with completing applications for public benefits assistance including SNAP, Medicaid, TANF, and SSI&nbsp;<br>Follows up with patients and addresses barriers to accessing community programs and medical services&nbsp;<br>Manages an assigned caseload of patients&nbsp;<br>Demonstrates empathy and provides emotional support to patients and families&nbsp;<br>Acts as a patient advocate and liaison between the patient/family, community service agencies, and the clinical care team&nbsp;<br>Documents clinical encounters in an electronic medical record and tracks outcomes&nbsp;<br>Builds trusting relationships with patients, families, clinical team members, and community partners&nbsp;<br>Collaborates with other members of the care team&nbsp;<br>Develops and maintains key community partnerships&nbsp;<br>Continuously expands knowledge and understanding of community resources and services&nbsp;<br>Attends department meetings&nbsp;<br>Completes required trainings&nbsp;<br>&nbsp;</p><p><strong>Skills &amp; Requirements</strong></p><p>H.S. Diploma or GED required&nbsp;<br>Associate's degree Health Related field preferred&nbsp;<br>CHW - Community Health Worker by the Texas Department of State Health Services (DSHS) required&nbsp;<br>5 years Healthcare, nonprofit organization, government agency, or organization that provides direct assistance to individuals preferred &nbsp;&nbsp;</p>",
