Job Description: The System Flow Coordinator is responsible for providing single point of contact for clients within the Saskatchewan Heath Authority (SHA). Facilitating physician consults and patient transfers from facility to facility to ensure appropriate patient and system flow across the SHA. Collaborating with other stakeholders to complete repatriation of patients back to the appropriate home hospital. Coordinating the utilization of the electronic assessment system RAI-MDS (hereafter referred to as MDS) as the Screening tool for Continuing Care eligibility and placement and alternate level of care services placement within the SHA. Managing the placement of Continuing Care clients into the appropriate vacancy. Providing an effective program and continuous quality improvement and management practice to ensure efficient utilization of the system in consultation with Directors of assigned Integrated Service Areas. Maintaining and Reviewing the Acute Care Dashboard for Bed management purposes.
Human Resources Exemption: No
Experience
Demonstrated experience in MDS systems and interpreting MDS outcomes.
A minimum of (5) years in Continuing Care of Home Care
Combination of experience in acute/continuing/home care preferred
Five (5) years experience in Acute Care
5 years demonstrated experience in MDS systems
Education
Bachelor of Science in Nursing (BScN)
Licenses
Valid Class 5 driver's license
Other Education and Training
Previous Coordinator experience is an asset
Basic Life Support Certificate (BLS)
Nursing diploma is acceptable
Bachelor of Psychiatric Nursing is acceptable
Psychiatric Nursing diploma is acceptable
