The OT utilizes <strong>GG (Functional Independence Measure &ndash; FIM/GG) scoring</strong> to assess patient progress and document functional outcomes in compliance with CMS (Centers for Medicare &amp; Medicaid Services) and other regulatory requirements.</p><p><strong>POSITION&rsquo;S ESSENTIAL RESPONSIBILITIES:&nbsp;</strong></p><p><strong>1. Patient Evaluation &amp; Treatment Planning:</strong></p><ul><li>Perform <strong>comprehensive initial and ongoing rehab assessments</strong> in <strong>IPR</strong>, focusing on ADLs, mobility, cognition, and discharge readiness.</li><li>Administer <strong>GG scoring (Section GG)</strong> for IPR patients per CMS guidelines (e.g., GG0130: Eating, GG0170: Sitting to Standing).</li><li>Use <strong>standardized assessments</strong> (e.g., MoCA, MMT, FIM) and evidence-based interventions to maximize recovery.</li></ul><p><strong>2. Treatment Planning &amp; Implementation:</strong></p><ul><li><strong>High-intensity ADL retraining (dressing, bathing, toileting).</strong></li><li><strong>Neuromuscular re-education</strong> for stroke/TBI recovery.</li><li><strong>Cognitive-perceptual rehab</strong> (memory, attention, executive function).</li><li><strong>Home assessments &amp; caregiver training</strong> pre-discharge.</li></ul><p><strong>3. GG Scoring &amp; Documentation:</strong></p><ul><li>Accurately complete <strong>Section GG functional scoring</strong> at admission, discharge, and per IRF-PAI guidelines.</li><li>Ensure compliance with:<ol><li><strong>CMS guidelines</strong> (Medicare Part A, IRF coverage criteria).</li><li><strong>Hospital policies</strong> (infection control, fall prevention).</li><li><strong>Joint Commission standards</strong> (safety, quality measures).</li></ol></li></ul><p><strong data-pasted=\"true\">4.</strong> <strong data-pasted=\"true\">Patient &amp; Caregiver Education:</strong></p><ul><li data-pasted=\"true\">Educate patients and families on home safety, fall prevention, and proper use of assistive devices.</li><li data-pasted=\"true\">Train caregivers in safe transfer techniques and mobility assistance.</li><li data-pasted=\"true\">Provide guidance on post-discharge exercises and adaptive strategies.</li></ul><p data-pasted=\"true\"><strong data-pasted=\"true\">5.</strong> <strong data-pasted=\"true\">Interdisciplinary Collaboration:</strong></p><ul><li data-pasted=\"true\">Participate in <strong>weekly team meetings</strong> to discuss patient progress and discharge planning.</li><li data-pasted=\"true\">Communicate effectively with physicians, nurses, case managers/social workers, and other therapists.</li><li data-pasted=\"true\">Adjust treatment plans based on patient response and medical status.</li></ul><p><strong>6.
