The employee also interacts with and takes direction from the PACE Lead PCA, interacts with others in the PACE Center. The employee has access to confidential information as necessary to perform job duties. Therefore, the employee is expected to respect the right to privacy and security of PHI (Protected Health Information) and EPHI (Electronic Protected Health Information). Positive and effective interpersonal/communication skills are required and are a high priority in this position.</p>\n<p> </p>\n<h2>Responsibilities</h2>\n<p><strong>Major Tasks, Duties and Responsibilities:</strong></p>\n<p><em>Note: “Essential functions” are primarily job duties that the employee must be able to perform unassisted or with some reasonable accommodation made by the company</em></p>\n<p>Essential Functions:</p>\n<ul>\n <li>Assists participants with their participation in activity programs at the PACE Center</li>\n <li>Sets up PACE center for snacks and lunches</li>\n <li>Set-up and clean-up program areas on a daily basis</li>\n <li>Ensure participants are able to participate in group activities and have the ability to interact with participants on an individual basis.</li>\n <li>Demonstrate the ability to communicate to the Center Manager any problems or needs in the program</li>\n <li>Assist with progress reports and routine assessments and other pertinent records as requested</li>\n <li>Abide by the rules and regulations of the PACE program</li>\n <li>Promptly report any accidents, incidents, unsafe and hazardous conditions and equipment and ensure that established safety rules and regulations are followed at all times</li>\n <li>Provides assistance at meal and snack times to participants including feeding as needed</li>\n <li>Provides personal care to participants at the PACE Center (bathing, toileting, incontinent care, feeding, transfers, lifting and ambulation)</li>\n <li>Provides laundering participants’ clothes and linens after bathing or personal hygiene care at the PACE center.</li>\n <li>Assists with ambulation, transfer and range of motion needs of participants as determined by PACE Physical and or Occupational Therapists.</li>\n <li>Notes changes in participants’ physical or emotional health and communicates this information to PACE HCC, RNs, or designee.</li>\n <li>Assist in the coordination of care delivery</li>\n <li>Obtains participant weights as requested</li>\n <li>Performs procedures / treatments under the supervision of Clinical RN.</li>\n <li>Completes documentation requirements in the PACE Center daily.</li>\n <li>Carries out the plan of care as directed.</li>\n <li>Participates in daily PCA meetings on personal care services provided and the self-care abilities of the participants and social interaction levels of participants and changes to plans of care</li>\n <li>Provides escort services for Participants to and from home and medical appointments</li>\n <li>Attends all mandatory in-services and PACE training as required</li>\n <li>Annually, successfully completes a competency skills test for the position</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong><u>Educational Requirements</u></strong><strong>: </strong></p>\n<ul>\n <li>High school diploma or GED</li>\n</ul>\n<p><strong><u>Experience:</u></strong> </p>\n<ul>\n <li>Some experience as an activity worker, homemaker, personal care aide, home health aide, or nursing assistant preferred.</li>\n <li>Minimum of one year of experience working with a frail or elderly population preferred.</li>\n <li>Discretion, tact, patience, and good judgment required.</li>\n <li>Empathetic and compassionate attitude in caring for elderly required.</li>\n <li>Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by Fallon Health Weinberg PACE Program and approved by CMS before working independently. </li>\n</ul>\n<p><strong><u>Immunizations:</u></strong> </p>\n<ul>\n <li>Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.</li>\n</ul>\n<p><em><strong>Pay Range Disclosure:</strong></em><em>In accordance with the Massachusetts Wage Transparency Act, the pay for this position is $ 19.00 per hour which reflects what we reasonably and in good faith expect to pay at the time of posting.
