    "description": "<p><span style=\"font-weight: bold\">Segue Health</span>-- <em>Faith Based Medical Practice</em></p>\n<p>Segue Health, a faith based medical practice, is seeking a compassionate <span style=\"font-weight: bold\">Care Coordinator</span> to join our team serving patients in the <span style=\"font-weight: bold\">Longview, TX </span>area<br>coordinating post discharge transitional care management. This position requires a mix of clinical knowledge, excellent organizational skills, and excellent communication skills.</p>\n<p><br><br></p>\n<p><span style=\"font-weight: bold\">Qualifications</span></p>\n<ul>\n<li>Must have experience in the medical field</li>\n<li>Must have a knowledge of Microsoft Excel and the ability to learn EMR/EHR software</li>\n<li>Should have transportation to and from local hospital </li>\n<li>Must have a heart to serve others</li>\n<li>Daily tasks include:\n<ul>\n<li>Working with referring facility to generate list of patients eligible for TCM services</li>\n<li>Visiting referrals while in the facility to explain the TCM program and determine if there are any anticipated needs post discharge</li>\n<li>Daily logging of referrals and discharges</li>\n<li>Creating charts in EMR and uploading face sheet, H&amp;P, and DC Summary</li>\n<li>Post discharge calls to patients within 24 hours</li>\n<li>Scheduling of patients preferably prior to discharge for in home visits </li>\n</ul>\n</li>\n<li>Our most successful candidates have experience in social work, nursing, and/or home health.</li>\n<li>Attention to detail and organizational skills are MOST important.</li>\n</ul>\n<p> </p>\n<p><span style=\"font-weight: bold\">Job Type: Full-time</span></p>\n<p><span style=\"font-weight: bold\">Salary:</span> $45,000.00 - $55,000.00 per year</p>\n<p><span style=\"font-weight: bold\">Competitive Benefits Provided</span></p>\n<p> </p>\n<p><span style=\"font-weight: bold\">Schedule:</span></p>\n<ul>\n<li>8 hour shift</li>\n<li>Monday to Friday</li>\n</ul>\n<p><span style=\"font-weight: bold\">About Segue Health</span></p>\n<p>Segue Health is a mission-focused organization dedicated to delivering exceptional, Christ-centered care to patients in their homes and communities.
