Apply today to join our team!</p>\n<p> </p>\n<p><strong>As a Home Support Supervisor you will:</strong></p>\n<ul>\n <li>Work with acute, chronic and palliative populations in partnership with the client and family to establish goals that are safe, realistic, and reasonable by assessing, planning, coordinating, organizing, implementing, evaluating, and transitioning social work and other services for the client in an interprofessional team model in collaboration with the primary care provider, and other care providers.</li>\n <li>Assess service eligibility, facilitate the development, implementation, and adaptation of a care plan, deliver direct care, and provide care management to assist clients to manage their own care and navigate through the various services available within the community setting.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education & Experience</strong></p>\n<ul>\n <li>Graduate of a recognized program for Licensed Practical Nurses.</li>\n <li>Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).</li>\n <li>Two years’ recent related experience working with geriatric clients, adults with chronic and acute illness including one (1) year in a supervisory role.</li>\n <li>Valid BC Class “V” Driver’s License. Local area travel requires the use of a personal vehicle.</li>\n</ul>\n<p><strong>Knowledge, Skills & Abilities</strong></p>\n<ul>\n <li>Demonstrated medication administration skills as outlined in the LPN Scope of Practice.</li>\n <li>Comprehensive knowledge of other health care disciplines and their role in client and family care.</li>\n <li>Knowledge and practice within the standards dictated by the BCCNM for LPNs.</li>\n <li>Broad knowledge of working with Medical, Surgical and Palliative clients.</li>\n <li>Demonstrated medication administration skills as outlined in the LPN Scope of Practice.</li>\n <li>Demonstrated ability to provide leadership and coach team members to promote an efficient and effective team based environment.</li>\n <li>Demonstrated ability to analyze and resolve problems efficiently and be resourceful.</li>\n <li>Demonstrated ability to adjust communication to suit the recipient and/or situation within a complex community environment.</li>\n <li>Demonstrated ability to communicate effectively both orally and in writing with coworkers, physicians, other health care staff, clients and their families, one to one and in groups.</li>\n <li>Demonstrated ability to work independently and in collaboration/consultation with others.</li>\n <li>Demonstrated ability to work under pressure and maintain priorities.</li>\n <li>Demonstrated skill in CPR techniques.</li>\n <li>Demonstrated word processing and database skills and computer literacy to operate a computerized client and scheduling information system.</li>\n <li>Physical ability to perform the duties of the position.</li>\n</ul>\n<h2>Closing Statement</h2>\n<p>The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.
