This engagement begins 120 days pre-release, and the support continues up to 90 days post-release to the community</li>\n\t<li>Working independently and as a team</li>\n\t<li>Assisting patients with transportation and housing resources and make referrals as needed</li>\n\t<li>Working on-site unless they are in the community working with patients.</li>\n\t<li>Coordinating&nbsp;necessary treatment for various patient needs and communicating&nbsp;with all referral sources and treatment entities using proper releases of information</li>\n\t<li>Working hours required to meet the business needs of the organization and travel to other programs or locations as needed</li>\n</ul>\n",
