We are currently seeking a Visit Nurse (RN or LPN) to join our ever growing team servicing Hancock county.&nbsp;</span></p><p><span style=\"font-size: 14px; font-family: verdana, sans-serif;\"><strong>Job Description</strong></span></p><ul><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">The Visit Nurse provides direct patient care in the home to adult and/or geriatric patients.&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Assesses the patient to identify physical, psycho-social, and environmental needs&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Communicates with the multidisciplinary team regarding patient progress&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Works under the direction of a Registered Nurse (RN) while demonstrating knowledge and technical skill necessary for providing competent, comprehensive, and age-specific patient care.&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Implements the plan of care initiated by the RN</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Provides nursing care within the parameters set forth by the physician&rsquo;s orders and RN assessment to ensure positive patient care outcomes.&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Documents patient progress and reports any significant changes in a timely manner to the supervising RN.&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Educates patients concerning procedures, treatments, and needs, and ensures medical orders are carried out accurately and in a timely manner.&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Provides patients with the knowledge and assistance to learn appropriate self-care techniques and information pertinent to diagnosis and plan of care.&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Assists in the administration of medications and treatments as prescribed by the physician&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Assures completeness of ordered procedures and treatments&nbsp;</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Provides accurate and timely documentation in accordance with the plan of care</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Participates in personal and professional growth and development</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Participates in on-call duties for assigned territory</li></ul><p><span style=\"font-size: 14px; font-family: verdana, sans-serif;\"><strong>Basic Qualifications</strong></span></p><ul><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Must be a graduate of an accredited school of nursing and holder of a current permanent or temporary license from the Indiana Medical Licensing Board</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Possess&nbsp;good verbal and written communication, and organization skills.</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Must possess an Indiana Driver&#39;s license and automobile liability coverage as required by the State of Indiana.</li><li style=\"font-size: 14px; font-family: verdana, sans-serif;\">Must have the ability to perform essential job functions with or without reasonable accommodation</li></ul><p><span style=\"font-size: 14px; font-family: verdana, sans-serif;\">Suburban Home Health is an Equal Opportunity Employer and prohibits discrimination and harassment of any kind.
