These assessments help determine functional status, care needs, and eligibility for long-term services.</p>\n<p> </p>\n<p>This position is ideal for RNs who enjoy assessment-focused work, independent field-based practice, and supporting complex populations through thoughtful evaluation and documentation. Unlike bedside roles, this position focuses on comprehensive patient assessments, clinical evaluation, and care coordination insight that directly supports care management teams and member service planning.</p>\n<h1>Special Project Opportunity</h1>\n<ul>\n <li>This role supports a full-time community health assessment project with Elderwood IPA for approximately three (3) months.</li>\n <li>Following the project, nurses may have the opportunity to transition into per diem RN opportunities through Elderwood Staffing Solutions, Elderwood’s internal float pool supporting facilities throughout Western New York. These opportunities provide flexible scheduling with the ability to pick up shifts ranging from occasional coverage to part-time or full-time hours. There may also be opportunity to </li>\n <li>While not required, this pathway offers the ability to continue working within the Elderwood network after the project concludes.</li>\n</ul>\n<p><strong>Registered Nurses with experience in community health, long-term care, or MLTC assessment work are encouraged to apply and support this important community health initiative with Elderwood IPA.</strong></p>\n<h2>Responsibilities</h2>\n<h1>What You’ll Do as RN UAS Assessment Nurse with Elderwood IPA:</h1>\n<h3>Conduct Comprehensive Community Health Assessments</h3>\n<ul>\n <li><p>Perform NYS UAS assessments in members’ homes or through virtual visits</p></li>\n <li><p>Evaluate functional status, medical needs, behavioral health factors, and community supports</p></li>\n <li><p>Complete required 6-month face-to-face reassessments</p></li>\n</ul>\n<h3>Clinical Evaluation & Care Insight</h3>\n<ul>\n <li><p>Identify changes in member condition, functional ability, and care needs</p></li>\n <li><p>Compare reassessment findings to baseline data to monitor health status and risk factors</p></li>\n <li><p>Recognize potential safety concerns or health deterioration and escalate appropriately</p></li>\n</ul>\n<h3>Collaboration & Care Coordination</h3>\n<ul>\n <li><p>Communicate findings with care management teams, primary care providers, and interdisciplinary partners</p></li>\n <li><p>Provide education to members and caregivers regarding chronic conditions, medications, and care planning</p></li>\n</ul>\n<h3>Documentation & Compliance</h3>\n<ul>\n <li><p>Complete detailed documentation in the electronic medical record and NYS UAS systems</p></li>\n <li><p>Ensure assessments meet DOH, Medicaid, CMS, and regulatory requirements</p></li>\n <li><p>Maintain compliance with HIPAA and Elderwood IPA policies</p></li>\n</ul>\n<h2>From</h2>\n<h2>Up to</h2>\n<h2>Qualifications</h2>\n<p><strong>RN UAS Assessment Nurse:</strong></p>\n<ul>\n <li><p>Current Registered Nurse (RN) license in New York State in good standing</p></li>\n <li><p>BSN, AAS, or Diploma in Nursing</p></li>\n <li><p>Minimum three (3) years of nursing experience</p></li>\n <li><p>At least one year of experience working with frail or elderly populations</p></li>\n <li><p>Experience completing NYS UAS assessments strongly preferred</p></li>\n <li><p>Valid New York State driver’s license</p></li>\n <li><p>Comfortable conducting in-home and virtual assessments</p></li>\n <li><p>Experience using EMR/EHR systems</p></li>\n <li>This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
