The<strong> SLP per visit range is $72.31 to $84.32</strong> and the <strong>hourly range is $36.15 to $42.15.</strong> Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.</em></p></li>\n</ul>\n<p> </p>\n<h2>What The Role Involves</h2>\n<ul>\n <li>Professional treatment of patients in the community and in their homes</li>\n <li>Delivers therapy within the Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome </li>\n <li>Assists with treatment plans, speech therapy and linguistics</li>\n <li>Works with the patient on appropriate time-specific, measurable goals and outcomes in collaboration with the care team</li>\n <li>Provides client/family education and support to address physical, functional and self-care needs as assigned</li>\n <li>Works independently and as an integral member of an interdisciplinary care team</li>\n <li>Monitors and report changes in health status to the supervising therapist</li>\n <li>Performs other clinical duties as required</li>\n</ul>\n<h2>What You Bring</h2>\n<ul>\n <li>Master’s Degree (or equivalent) degree in Speech-Language Pathology from a recognized university</li>\n <li>Current, unrestricted license to practice with the College of Audiologists and Speech-Language Pathologists of Ontario</li>\n <li>Current First Aid Certificate and Cardiopulmonary Resuscitation (CPR) Certificate</li>\n <li>Demonstrated understanding and practice of rehabilitation principles and techniques</li>\n <li>Sound critical thinking skills, good judgement and problem-solving skills</li>\n <li>Ability to participate and follow treatment programs and care plans</li>\n <li>Experience working one-on-one as a therapeutic support with clients</li>\n <li>Ability to foster independence of the client in the home and in the community</li>\n <li>Proven computer skills, including email, MS Word and Outlook, work scheduling systems</li>\n <li>A valid driver’s license and vehicle as regular travel is required</li>\n <li>Excellent English verbal and written communication skills</li>\n <li>To protect our patients, we require current vulnerable sector check screening, and a clear background check</li>\n</ul>\n<h2>CarePartners In Your Community</h2>\n<p>In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings.
