Assure residents balance nutrition needs due to specific medical conditions with food preferences to achieve agreed-upon goals.</p><p><strong>Qualifications &amp; Education Required:</strong></p><ul style=\"list-style-type: disc;\"><li>Bachelor&rsquo;s Degree in Food and Nutrition or Dietetics required</li><li>ADA registered or registration eligible required</li><li>Certified Dietitian/Nutritionist in NYS preferred</li><li>1 year in long-term care preferred</li><li>Ability to interact with elders and care for their needs</li></ul><ul type=\"disc\"><li>Ability to lead and coordinate team activities</li></ul><p><strong>Position Competencies:</strong></p><ul style=\"list-style-type: disc;\"><li>Interpersonal skills: support the emotional, physical, and social well-being of elders and Core Team</li><li>Problem solving/decision making</li><li>Technical expertise in Matrix Care, MS Office, and BluePrint</li></ul><p><strong>Position Responsibilities:</strong></p><ul style=\"list-style-type: disc;\"><li>Responsible for nutritional assessments and monitoring of assigned residents</li><li>Provides direction, leadership, and coaching to Dieticians on the team. This will include caseload coverage, as well as time off requests. They will provide updates to the team on clinical nutrition process changes, DOH updates, MDS changes, etc.</li><li>Participate in QAPI activities including data collection, audits, monitoring, and quality improvement plans that impact clinical nutrition outcomes</li><li>Collaborate with clinical teams to facilitate appropriate nutritional care and care planning on an as needed basis</li><li>Provide diet counseling to residents and their family members as needed</li><li>Act as a liaison for policy and procedures pertaining to clinical nutrition</li><li>Serve as a liaison between foodservice and clinical nutrition needs; including menu updates, product updates and changes and coordination of therapeutic and mechanically altered dietary needs</li><li>Responsible to serve in MOD role as assigned</li><li>Attend All-managers meeting</li><li>Help with other duties as needed that support the Core Team to fulfill St John&rsquo;s mission vision and values</li></ul><p><br></p><p><strong>Physical Requirements:</strong></p><ul style=\"list-style-type: disc;\"><li>Walking, standing for lengthy periods of time.</li></ul><p><br></p><p><strong>Exposure to Conditions:</strong></p><ul style=\"list-style-type: disc;\"><li>May provide care to individuals with infectious diseases.
