Description

Overview The Integrated Outpatient Coordinator provides leadership and clinical oversight for Chestnut’s adult Integrated Outpatient program, delivering both substance use and mental health treatment services. This role oversees program operations, staff supervision, service quality, and coordination of care while ensuring timely access to services and adherence to evidence-based, recovery-oriented, and trauma-informed practices. The Coordinator works collaboratively across departments to support seamless client engagement and high-quality integrated care. We provide fully integrated care for all our patients by combining behavioral health care services with our community-based primary care health centers. Chestnut also conducts research and training throughout the U.S.A. This role is Student Loan Repayment Eligible. National Health Service Corp (NHSC) LOAN REPAYMENT PROGRAM - The NHSC LRP offers up to $100,000 for providers working in Health Professional Shortage Areas (HPSAs) in Illinois. Responsibilities Supervise the adult Integrated Outpatient program in an assigned county, providing both substance use and mental health treatment services, including assessment, individual, family, group, and Immediate Access services Oversee the development, planning, implementation, and evaluation of services in accordance with governing policies, procedures, and quality standards Model and reinforce evidence-based practices, including recovery-oriented care, trauma-informed care, Motivational Interviewing, and the Community Reinforcement Approach Provide leadership and direction to promote positive collaboration within Chestnut and with community partners, particularly related to internal referrals and coordinated care Hire, train, supervise, and evaluate staff, interns, and volunteers, including providing certification and funder-required supervision as applicable Collaborate closely with the Centralized Access department to ensure timely client access, efficient scheduling, and continuity of care Provide direct clinical services at an assigned level and ensure staff meet expected service delivery levels Promote Chestnut’s culture of customer service excellence through adherence to behavioral standards Maintain confidentiality and security of all organizational, financial, and consumer information Perform other duties as assigned to support program and agency goals Qualifications To support a broad and diverse candidate pool, Chestnut welcomes applicants who meet one or more of the following qualification pathways: Bachelor’s degree in a behavioral health field, education, or social sciences (e.g., Community Health, Sociology, Community Psychology) with a minimum of five (5) years of human services experience OR Master’s degree in a human services or behavioral health–related field Certification as a Certified Alcohol and Drug Abuse Counselor (CADC) required AND / OR (for advanced scope candidates): Master’s degree in a human services field with licensure as an LMFT, LCSW, or LCPC Minimum of three (3) years of clinical experience, with both mental health and substance use treatment experience preferred Additional Requirements (all levels): Supervisory experience preferred Strong organizational, verbal, and written communication skills Basic computer proficiency, including Microsoft Word, Excel, Outlook, email, and electronic medical record (EMR) systems Are you intrigued by this job but don’t meet every single requirement?
