Serving approximately 19,534 annually and providing 1,839 units of supportive housing last year, LOFT is one of Ontario’s largest mental health service providers of its kind. LOFT is also recognized as a Nonprofit Employer of Choice™ Award (NEOC) recipient. Job Summary: The coordinator provides overall team leadership and ensures the effective delivery of harm reduction and outreach services across the regions. This role oversees day-to-day operations, including Case Manager services, and supports clients by facilitating optimal care through a range of assistance and resources. In collaboration with the Manager, the Coordinator develops and maintains relationships with network partners, represents LOFT at system tables, and ensures staff including partner agency staff are informed about program operations and documentation requirements for reporting purposes. The coordinator is also responsible for guiding staff, maintaining service quality, and fostering strong relationships with individuals who are unhoused, at risk of homelessness, street-involved, or experiencing mental health and substance use challenges. The LOFT Assisted Living Hub and Spoke Orillia program provides specialized support to vulnerable and at-risk older adults with complex physical health challenges, mental health challenges, addictions or cognitive impairment. The program supports each client’s individual needs so they can live safely and in good health in the community as they continue to age. The program helps provide stability to individuals in the surrounding community, helps reduce unnecessary or avoidable ED visits and reduces the length of stay in hospital. The program works with each client on an individualized client care plan based on their wants, needs, and strengths. The LOFT Assisted Living Hub and Spoke Orillia program is a community-based program that provides care coordination, case management and personal support. The team can also provide access to specialized services for mental health and addictions and emergency response planning and equipment. What You Will Do: Site Management & Compliance: Oversee all aspects of site management, including maintenance, security, and ensuring adherence to health, safety, and regulatory standards.. Client Support & Enpowerment: Promote client independence and well-being through coaching, mentoring, and advocacy, helping clients access services, resolve conflicts, and navigate social and healthcare systems. Team Leadership & Staff Coordination: Provide direction to front-line staff, including scheduling, training, recruitment support, and performance management to ensure effective service delivery. Operational Monitoring & Administration: Manage daily operations, maintain accurate records, oversee petty cash, coordinate repairs and resources, and ensure proper documentation and incident reporting. Program Planning & Evaluation: Contribute to service planning, quality improvement, and program evaluation activities, including reporting, data tracking, and identifying service gaps. Client Engagement & Community Building: Facilitate client meetings and group activities that support well-being and a positive living environment. On-Call Support: Participate in a rotating after-hours on-call schedule to ensure continuous operational support. What You Bring: A degree/diploma in health care/social services or another related field At least 5 years’ relevant experience in the mental health sector. Strong understanding of harm reduction and Psychosocial Rehabilitation (PSR) principles, with the ability to apply these in practical, client-centered settings. Strong intake assessment, crisis intervention, advocacy, and problem-solving capabilities. Experience supporting individuals with complex needs, including mental health challenges, addictions, cognitive impairments, and other vulnerable populations. Experience in program development, planning, evaluation, and facilitating groups or meetings. Excellent communication, counselling, and relationship-building skills, with the ability to engage clients in sensitive and complex situations. Familiarity with community resources and ability to build effective relationships with service providers, families, and support networks. Computer literacy for documentation, reporting, and maintaining records. A valid Ontario driver’s license may be required. Work Environment: Frequent bending, kneeling and reaching, combined hours of sitting and standing while using office equipment and computers, light lifting of supplies and materials from time to time. Lifting to 50 pounds, exposure to elements when working mobile outreach shifts, sitting and/or driving for extended periods of time, May be required to be on-call as emergency contact for after hours. Staff will be required to work in client rooms and may be exposed to secondhand smoke.
