    "ExternalDescriptionStr": "<p><strong>Centra PACE (Program of All-inclusive Care for the Elderly</strong>) is a program that provides comprehensive health care, including all wrap-around services, to eligible participants. Eligible participants are 55+ and qualify for nursing home placement. They must be safe to live in the community with PACE support. &nbsp;</p><p>Each individual enrolled will work with a team of caregivers to create a personalized plan of care based on their goals, values and preferences.
