Your role is crucial in helping patients—particularly those in underserved communities—access and benefit from these services. You will coordinate patient care, facilitate communication between healthcare providers and patients, verify insurance eligibility, and maintain accurate patient records. By ensuring timely interventions and follow-ups, you will contribute to improved patient outcomes and reduced hospital visits.</span><br></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Key Responsibilities:</span></p>\n<ul>\n<li><span>Assist patients in enrolling in RPM, CCM, and CHI programs, providing clear guidance throughout the process.</span><br></li>\n</ul>\n<ul>\n<li><span>Schedule and manage patient appointments, including enrollments, escalations, follow-ups, and healthcare visits such as M11q and DOH-type appointments.</span><br></li>\n</ul>\n<ul>\n<li><span>Collaborate with healthcare teams to ensure seamless coordination and delivery of care across RPM, CCM, and CHI programs.</span><br></li>\n</ul>\n<ul>\n<li><span>Ensure accurate and timely data entry into EMR systems (e.g., AdvancedMD) and other platforms, ensuring compliance with HIPAA regulations.</span><br></li>\n</ul>\n<ul>\n<li><span>Monitor and manage patient referrals, leads, and opportunities in Salesforce, maintaining accuracy and following up as needed.</span><br></li>\n</ul>\n<ul>\n<li><span>Address patient inquiries and escalate concerns related to RPM, CCM, and CHI services to appropriate healthcare professionals.</span><br></li>\n</ul>\n<ul>\n<li><span>Proactively follow up with patients to ensure engagement with their care plans, maintaining regular communication with patients, caregivers, and providers.</span><br></li>\n</ul>\n<ul>\n<li><span>Conduct insurance eligibility checks via platforms such as e-PACES, verifying coverage for RPM, CCM, and CHI services.</span><br></li>\n</ul>\n<ul>\n<li><span>Enhance provider efficiency by performing pre-appointment cross-checks in ChronicCareIQ (CCIQ) and HHA Exchange, ensuring patient charts are verified, updated, or entered accurately.</span><br></li>\n</ul>\n<ul>\n<li><span>Maintain effective, professional communication with patients and team members through phone, email, and other correspondence, fostering a supportive and welcoming patient experience.</span><br></li>\n</ul>\n<ul>\n<li><span>Oversee device ordering and setup for RPM, ensuring patients receive and understand how to use their devices for effective monitoring.</span><br></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Required Qualifications:</span></p>\n<ul>\n<li><span>High school diploma required.</span><br></li>\n<li><span style=\"font-weight: bold\">Bilingual fluency in Spanish is required.</span><br></li>\n</ul>\n<ul>\n<li><span>Previous experience in sales, ideally in a healthcare setting.</span><br></li>\n</ul>\n<ul>\n<li><span>Proficiency in using RPM and CCM platforms (e.g. ChronicCareIQ) and EMR systems (e.g., AdvancedMD).</span><br></li>\n<li><span>Experience with Salesforce for managing referrals, leads, and patient opportunities.</span></li>\n</ul>\n<ul>\n<li><span>Strong verbal and written communication skills, with the ability to handle sensitive patient interactions empathetically and professionally.</span><br></li>\n</ul>\n<ul>\n<li><span>Excellent organizational and multitasking abilities, with attention to detail in scheduling, data entry, and patient follow-ups.</span><br></li>\n</ul>\n<ul>\n<li><span>Proficiency in telehealth platforms, communication tools (e.g., Zoom), Microsoft Office, and HIPAA compliance in virtual environments.</span><br></li>\n</ul>\n<ul>\n<li><span>Strong problem-solving skills and the ability to work effectively in a fast-paced, virtual environment.</span><br></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Preferred Qualifications:</span></p>\n<ul>\n<li><span>Additional education or certification in Medical Administration is preferred.</span></li>\n<li><span>Familiarity with HHA Exchange for coordinating patient care is preferred.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Benefits:</span></p>\n<ul>\n<li>Health, dental, and vision insurance</li>\n<li>401(k) with company match</li>\n<li>Paid Vacation, Company Holidays, Sick Leave, and Floating Holidays<br></li>\n<li>Professional development opportunities</li>\n<li>Mission-driven work in an innovative healthcare space</li>\n</ul>",
