Assess, develop, implement and evaluate an individualized plan of care to maximize the comfort and health of the Hospice and Palliative Care patient and family. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending Physician�s orders, Plan of Care and Hospice and Home Care policies and procedures.\n<h2>Qualifications</h2>\n<ul>\n <li>Graduate of an NLN accredited school of Nursing is required.</li>\n <li>Bachelor of Science in Nursing is preferred.</li>\n <li>Two years recent hospital, medical/surgical or other relevant nursing experience required.</li>\n <li>Hospice & Palliative Care Certification is preferred.</li>\n <li>Demonstrated knowledge of I.V. pump, and central line maintenance preferred.</li>\n <li>Has knowledge and skills necessary to provide care, which is appropriate to the population of patients served.</li>\n <li>Proficient computer skills, including experience with various software, database and electronic records programs is preferred.</li>\n <li>Current RN licensure in the Commonwealth of MA and RI is required.</li>\n <li>Hospice & Palliative Care Certification will be required within eligibility timeframe of 2 years if not certified at time of hire.</li>\n <li>Basic life support (BLS) certification is required.</li>\n <li>Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired</li>\n</ul>\n<p> </p>\n<p> </p>\n<p><em><strong>Compensation: Pay rate will be determined based on level of experience. </strong></em></p>\n<h2>Pay Range</h2>USD $37.82 - USD $53.96 /Hr.",
