Clients will meet with their Case Manager on a weekly basis and attend daily psychoeducational groups. The program is designed to support women, as they return to their communities as a healthy and productive parent in recovery.</p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><u>Position Summary:</u></p>\r\n\r\n<p>The Case Manager 1 is responsible for delivering case management services to the Women &amp; Children Substance Use Treatment Facility. Duties include intake assessments, development and implementation of individual care plans, liaison with treatment providers and community resources, facilitation and documentation of client and group activities as well as daily psychoeducation groups in accordance with Women&rsquo;s and Children&rsquo;s curriculum and DMHAS core topics.</p>\r\n\r\n<p>The Case Manager 1 is a member of the care team providing care and comprehensive substance use treatment services to women who have young children or are expecting a child, at our residential Women and Children Substance Abuse Treatment facilities.</p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><u>Requirements:</u></p>\r\n\r\n<ul>\r\n\t<li>Some post secondary education</li>\r\n\t<li>2 years relevant experience</li>\r\n\t<li>Valid Connecticut Driver&rsquo;s License</li>\r\n\t<li>Regular and predictable attendance is required</li>\r\n</ul>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><u>Orientation: </u></p>\r\n\r\n<p>If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.</p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><u>Benefits:</u></p>\r\n\r\n<ul>\r\n\t<li>Medical, dental, vision benefits are offered along with other additional voluntary coverages with full time employment</li>\r\n\t<li>403(b) retirement plan with employer matching contribution</li>\r\n\t<li>Company paid short and long term disability and life insurance with full time employment</li>\r\n\t<li>Paid time off (vacation, personal and sick) with full time employment</li>\r\n\t<li>12 paid holidays</li>\r\n</ul>\r\n\r\n<p><em>The Connection is a statewide human services and community development agency that provides unique solutions to the problems of homelessness, mental illness, substance use, improving child welfare and community justice rehabilitation.
