They are not intended as a complete list of all responsibilities, duties and skills required of personnel so classified&nbsp;</span></p><p><strong>SUMMARY:</strong>&nbsp;</p><p>The Respite Care Provider provides short term, direct care for eligible participants. The purpose of this care is to provide relief for family members, parents or guardians to enable them to attend to personal matters. The Respite Care Provider has a demonstrated interest in working in direct care position serving the emotional, physical and health needs of eligible participants.&nbsp;</p><p><strong>DUTIES AND RESPONSIBILITIES:</strong>&nbsp;</p><p style=\"text-align:justify;\">1. Depending upon the eligible participants needs provides direct care as per caregiver instruction indicated on the Care Provision Instruction form. This may include assisting with in home activities of daily living.&nbsp;</p><p style=\"text-align:justify;\">2. Assures that health and safety needs are observed.&nbsp;</p><p style=\"text-align:justify;\">3. Prepares meals and assists the participant in nutritional skills, ensuring that nutritional needs are met within the home during the time of care.&nbsp;</p><p style=\"text-align:justify;\">4. Observes and supervises eligible participants in home activities.&nbsp;</p><p style=\"text-align:justify;\">5. Performs or requests emergency assistance as required.&nbsp;</p><p style=\"text-align:justify;\">6. Maintains records and prepares case notes daily on services provided within the home.&nbsp;</p><p style=\"text-align:justify;\">7. Maintains current knowledge of Respite Program services and technology.&nbsp;</p><p style=\"text-align:justify;\">8. Inputs participant information and case notes into electronic medical record, in accordance with agency procedures, state and federal requirements.&nbsp;</p><p style=\"text-align:justify;\">9. Implements all agency policies as appropriate.&nbsp;</p><p style=\"text-align:justify;\">10. Attends training seminars, in-service sessions, and staff meetings.&nbsp;</p><p style=\"text-align:justify;\">11. Responsible for maintaining training compliance in accordance with FRA policies and procedures, state and federal regulations. &nbsp;</p><p style=\"text-align:justify;\">12. Provides other support services and duties as necessary.&nbsp;</p><p style=\"text-align:justify;\">&nbsp;<strong>MINIMUM EDUCATION AND EXPERIENCE:&nbsp;</strong>&nbsp;</p><p style=\"text-align:justify;\">High School Diploma or equivalent. &nbsp;</p><p style=\"text-align:justify;\">1 &ndash; Year Verifiable Work Experience (Preferred experience in Human/Social Services Setting).&nbsp;</p><p style=\"text-align:justify;\">&nbsp;Must meet agency insurance criteria for operating agency and personal vehicles on agency business (21 y/o and licensed 3 years).&nbsp;</p><p style=\"text-align:justify;\">&nbsp;<strong>KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:</strong>&nbsp;</p><p style=\"text-align:justify;\">Demonstrate interest in working in a direct care position serving the emotional, physical and health needs of eligible participants. Demonstrate proficiency in direct care activities and training. Ability to communicate effectively with participants and families, assess needs and coordinate individual participant services. Ability to work a flexible schedule. Proficient in menu planning, meal preparation and general housekeeping functions. Must have reliable transportation and a good driving record. Must possess the knowledge and ability to utilize a computer and electronic medical record (EMR) system. Those activities which demand detailed or important instructions spoken to other workers accurately, loudly or quickly.&nbsp;</p><p style=\"text-align:justify;\">Hearing: Perceiving the nature of sounds with no less than a 40 lb loss.
