We look forward to learning more about you and exploring how your skills and passion for patient care can contribute to our team.</p><p><strong>Position Summary:</strong></p><p>As part of the integrated health model of Heart City Health Center, the position provides integrated behavioral health services to patients currently receiving primary care services at HCHC. The Licensed Mental Health Therapist works as an interface for patients accessing services such as individual, peer/family counseling and support, screening, assessment, brief treatment, consultative services, and education. This position works directly with primary care providers to strengthen relationships between behavioral health and primary care services at Heart City Health Center.&nbsp;</p><p><strong>Essential Functions:</strong></p><ul style=\"list-style-type: disc;\"><li>Comply with established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and customer service standards.&nbsp;</li><li>Provide ongoing consultation to primary care services for appropriate mental health interventions.</li><li>Participate in ongoing, current behavioral health education, training, and skills.&nbsp;</li><li>Provide training to primary care clinic staff in behavioral health topics including, but not limited to, counseling skills and confidentiality, substance use and misuse, including medication, wellness, and self-care, self-sufficiency, rehabilitation, and recovery.&nbsp;</li><li>Ability to conduct brief, non-stigmatizing screenings to evaluate the needs of adults, adolescents, and children for behavioral health and support services. The Evaluations based on medical record, staff, direct interview, collateral information and other reliable secured sources legally allowed.&nbsp;</li><li>Ability to perform comprehensive, bio-psychosocial assessment for mental health, substance use disorder, co-occurring disorders, domestic violence and medical needs. Conduct follow-ups based on results of evaluation as needed.&nbsp;</li><li>Provide early interventions, referrals to specialty mental health services, referral to more intensive psychiatric/psychological as needed.</li><li>Electronic medical record system (EMR) documentation according to established protocol, state/and national regulations, document findings in patient&rsquo;s medical record according to established protocol.&nbsp;</li><li>Participation in monthly Provider meetings.&nbsp;</li><li>Provide consultation and case by case consultation on behavioral/mental health issues affecting patients, as needed.&nbsp;</li><li>Provide training to primary care staff on diagnosis and treatment, medication management, confidentiality, and other behavioral health topics as needed.&nbsp;</li><li>Participate with other staff in multidisciplinary staff meetings, retreats and other staff processes to achieve program development, staff development, program evaluation, program operations review, and other activities needed to maintain quality of early intervention services.&nbsp;</li><li>Maintain confidentiality regarding privileged administrative and client information in a professional manner.&nbsp;</li><li>Other duties as assigned.&nbsp;</li></ul><p><strong>Knowledge, Skills and Abilities</strong>:</p><ul style=\"list-style-type: disc;\"><li>Excellent communication skills; active listening, as well as written and oral comprehension/communication skills; Gives full attention to what individuals are saying, understands the point being made, asks appropriate questions to gain better knowledge of situation(s) and repeats information to ensure understanding; bilingual (English/Spanish) language skills helpful</li><li>Excellent customer service skills; actively seek ways to assist internal and external customers within the scope of assigned duties</li><li>Good computer skills; Outlook, Windows, Microsoft Office applications</li><li>Good time management skills; self-evaluates the use of time and understands how others may be affected</li><li>Cultural diversity awareness and skills; respects all people regardless of race, nationality or social standing</li><li>Ability to work independently (self-motivating) and as a team member</li><li>Ability to develop a collaborative therapeutic alliance with individuals and make accurate professional judgments</li><li>Ability to build and maintain effective working relationships with co-workers, providers, managers, patients, agency resource personnel and community members in general</li><li>Familiarity with local community resources for patients with chronic disease</li><li>Knowledge of the health and human services infrastructure, health insurance programs and public coverage options</li><li>Problem sensitivity skills; empathetic/understanding</li><li>Deductive reasoning and problem-solving skills</li><li>Organized and detail-oriented</li></ul><p><strong>Education, Experience and Licensure:</strong></p><ul style=\"list-style-type: disc;\"><li>A Licensed Mental Health therapist (LMHT), LMHC, LMFA, (LCSW) required.</li><li>Experience (2 years or more preferred) serving clients with behavioral disorders, psychological illness, and /or substance use disorders.
