Assists patient and family in understanding, coping and adapting to the personal, emotional and bio-psychosocial issues associated to the patient's illness. Provides comprehensive assessments and referrals, crisis intervention and care management. Collaborates with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. May provide functional direction to unlicensed social work staff and social work interns.</p>\n<h2>Qualifications</h2>\n<strong>Education</strong>\n<ul>\n <li>Master's Degree in Social Work required</li>\n</ul>\n<strong>Experience</strong>\n<ul>\n <li>1 year of clinical, case management, social work or related experience required</li>\n</ul> \n<strong>Licenses and Certifications</strong>\n<ul>\n <li>Current Basic Life Support for Healthcare Providers by American Heart Association required</li>\n</ul>\n<p> </p>\n<h2>Disclaimers</h2>\n<p>• Pay ranges listed are an estimate and subject to change.• If any bonuses are noted, they are only applicable to external hires meeting criteria.</p>",
