The Occupational Therapist is responsible for ensuring and coordinating quality and safe delivery of home health occupational therapy services following applicable licensure/certification requirements both State and Federal. The Occupational Therapist has responsibility for the planning and therapeutic interventions of all occupational therapy cases. &nbsp; Occupational Therapy will be provided by a licensed Occupational Therapist or a licensed Occupational Therapy Assistant under the supervision of a licensed Occupational Therapist in compliance with the Plan of Care.</p><p><strong>&nbsp;PRN Opportunity!</strong></p><p><strong><u>Contributions:</u></strong></p><ul><li>Evaluates the patient&rsquo;s needs, including medical, psychosocial and environmental needs.&nbsp;</li><li>Develops a clinical assessment based on evaluation/re-evaluation findings and requisition and management of equipment and supplies required to increase the clients function and independence as appropriate to care.&nbsp;</li><li>Develop and implements, with physician and patient, individualized Plan of Care based on patient&rsquo;s physical, medical, psychosocial and environmental needs, and makes appropriate revisions as the patient&rsquo;s condition changes. &nbsp;</li><li>Establishes appropriate realistic and measurable goals for patients to achieve, then evaluates progress towards these goals.&nbsp;</li><li>Obtains initial and interim physician&#39;s orders and reports on progress and changes in client condition to physician and Clinical Manager.&nbsp;</li><li>Provides Occupational Therapy treatments consisting of active range of motion/modalities, therapeutic exercise and ADL retraining in accordance with the plan of treatment.&nbsp;</li><li>Performs reassessment and case review as appropriate and timely revision of the individualized care plan.&nbsp;</li><li>Provides case management including planning, coordination of services, and therapeutic interventions as required, in the absence of skilled nursing.&nbsp;</li><li>Supervises the Occupational Therapy Assistant through:</li><li>Instructions on initial care plan as established by the qualified Occupational Therapist.&nbsp;</li><li>Performs supervisory visits as required by the state and through review of documentation and communication.&nbsp;</li><li>Provides timely documentation of supervision of the certified therapy assistant.&nbsp;</li><li>Supervises the Home Health Aides through:</li><li>Prepares the home health aide assignment sheet/written instructions indicating care and tasks to be performed.</li><li>Communicates regularly with the Home Health Aide</li><li>Performs and documents Home Health Aide supervisory visits no less than every 14 days to evaluate:<ul><li>Evaluated effectiveness of the Home Health Aide care provided</li><li>Rapport with patient/family</li><li>Infection control procedures</li><li>Honoring patient rights&nbsp;</li><li>Following Home Health Aide assignment sheet&nbsp;</li></ul></li><li>Revises Home Health Aide assignment sheet/written instructions as necessary</li><li>Teaches and supervises Home Health Aides in new non-skilled tasks and procedures and documents such training.</li><li>Completes comprehensive OASIS Recertification/Follow &ndash;Up, if Occupational Therapy is the only discipline ordered, along with other recertification paperwork as appropriate.&nbsp;</li><li>Completes a comprehensive OASIS Resumption of Care Assessment along with other resumption documentation, as appropriate, if Nursing is not in on the case.&nbsp;</li><li>Completes a comprehensive OASIS Discharge Assessment if last skilled discipline providing service.</li></ul><p><br></p><p><strong><u>Job Requirements:</u></strong></p><ul><li>Excellent oral and written communication skills in the English language required</li><li>During emergencies or natural disasters may be required to be on duty for the duration of the disaster.</li><li>Is a graduate of an Occupational Therapy program and is licensed in the state of Florida.</li><li>A current Florida driver&rsquo;s license, clean driving record and proof of insurance are required.</li><li>Has 2 years&rsquo; experience preferred</li><li>Previous geriatric and home health experience preferred</li></ul><p>&nbsp;</p><p>Moorings Park &reg; Communities, a renowned Life Plan organization includes three unique campuses located in Naples, Florida.
