This field-based role focuses on engaging adults experiencing homelessness and serious mental illness and connecting them with housing, healthcare, and essential community services.</p><p style=\"margin-left: 0in; line-height: 1.5;\">As a Case Manager, you will build relationships with individuals in the community, coordinate services with partner agencies, and help clients overcome barriers to stability and recovery.
