POSITION DESCRIPTION Reentry & Recovery Case Manager Exemption Status: Non-exempt Pay Grade(s): 9/10 ($30.26 - 39.47+ DOE) Purpose: The Reentry & Recovery Case Manager is a key provider in the Reentry & Recovery program, serving participants who live in the non-congregate shelters reentering the community from incarceration or treatment, or seeking a safe and sober living environment. The incumbent provides essential case management services including coaching, mentorship and guidance for the Reentry & Recovery Program participants. The incumbent uses a wrap-around style service approach to provide professional and comprehensive casework services. This position also assists the Participant Services Manager in the oversight and general guidance of R&R Peer Support Specialists. Essential Functions. An individual in this role must be able to perform the following functions with or without reasonable accommodation: Complete strengths and needs assessment with each participant at time of program admission and throughout their time in program, as indicated. Develop person-centered, individualized case plans with goals and objectives based on strengths and needs identified in assessment and through ongoing services. Provide professional and comprehensive case management services including linking, planning, advocating and monitoring of case plans for assigned program participants. Work with participants to develop individualized exit plans, regularly reviewing and planning for successful community reentry that includes employment and permanent housing. Engage appropriately with returning citizens to support them in identifying existing supports and community resources available. Work closely with Probation/Parole Office and participants to support compliance with terms of probation/parole. Work with each participant on their financial wellness to create a financial plan. Work with the R&R Operations Manager to ensure that required payments are being made to the Reentry & Recovery Program. Ensure each participant’s right to privacy and maintain strict confidentiality at all times. Participate in case staffing reviews with Participant Services Manager, including regularly reviewing case plans and exit plans for each participant. Maintain thorough and timely case records within electronic record keeping system, including documenting all contact with participants and service providers. Responsible for interdepartmental and interagency coordination of services Responsible for learning about potential referral programs Responsible for working with referrals from departments across Tlingit & Haida, Dept. of Corrections (DOC), and various treatment centers Assist participants with access to community resources, including public transit, Job Service, Alask Learning Network/SERRC, local 12 step/support meetings and/or other recovery supporting meetings, mental health services, and other community resources. Interview possible program participants and make recommendations as to appropriateness of applicant as a participant of the Reentry & Recovery Program. Be available to do in-reach education about the Reentry & Recovery Program at LLLC (Lemon Creek Correctional Center) Explain program options and services available to participants and the requirements for each program.
